Job Type Full-time Description POSITION TITLE: Program Coordinator (OneCare Kansas) FLSA CLASSIFICATION: Non-Exempt REPORTS TO: Sr. Director Adult and Children Services POSITIONS SUPERVISED: OneCare Kansas Care Manager POSITION OVERVIEW: The One Care Kansas (OCK) Coordinator will work with the One Care Kansas (OCK) Team, Adult and Children's Case Management, Counseling Center staff, and Residential program to recruit, enroll and coordinate cases for OCK. The Coordinator is responsible for supervising staff who will coordinate physical and behavioral health services and supports, with emphasis on linkages to community and social supports. ESSENTIAL POSITION RESPONSIBILITIES:

  • Leads a staff of employees. Motivates, coaches, and supports employees toward maximizing their performance and productivity.
  • Plans and implements the orientation and training of employees per program guidelines.
  • Coaches and collaborates with staff to problem-solve barriers to completing daily, weekly, and monthly job requirements
  • Handles personnel matters effectively and consistently with existing policies, including interviewing, hiring, counseling, terminating, and completing annual evaluations for employees. Provides appropriate documentation.
  • Ensures timely and appropriate documentation and billing of services in the manner required by MHA, contractual and payer sources.
  • Leads recruitment of new OCK members to MHA from internal referrals and external resources, such as doctors offices, specialty clinics, hospitals, behavioral health providers, and/or schools. Collaborates with programs across the agency to identify eligible clientele.
  • Ensures Health Action Plans (HAPS) are completed timely.
  • Prepares and submits reports in accordance with contract and agency requirements.
  • Collects and tracks data for all program components to ensure accurate assessment of project outcomes.
  • Develops innovative strategies to ensure success and participation in the various programs.
  • Evaluates consumer satisfaction with services.
  • Attends and participates in internal and external meetings as indicated and required of program.
  • Participates in quality assurance and utilization review measures.
  • Updates program plans to ensure conformance with CARF and Medicaid guidelines.
  • Maintains and updates ongoing client rosters to ensure adequate client-to-staff ratios
  • Engages in regular communication with MCOs to foster positive working relationships, coordinate on client care, and complete yearly audits.
  • OTHER POSITION REQUIREMENTS:
  • Maintains acceptable overall attendance record, to include department staff meetings, agency meetings, and trainings as required. Ensures appropriate notification to supervisor for absences, and ensures that work is covered. Flexible in work schedule when needed.
  • Exhibits appropriate level of technical knowledge for the position.
  • Produces quantity of work necessary to meet job requirements.
  • Works well with a team, keeps others informed of information needed. Treats others with respect, maintaining a spirit of cooperation.

  • Completes assigned work, meets deadlines without reminders/follow-up from supervisor or others.
  • Performs work conscientiously with a high degree of accuracy.
  • POSITION REQUIREMENTS: The OCK Coordinator is expected to have a Bachelor's Degree in the Human Services field or equivalent. The Coordiantor should demonstrate effective communication skills and is expected to have a professional manner when dealing with staff, consumers, public and volunteers. A valid Kansas drivers license and access to personal vehicle required. PHYSICAL REQUIREMENTS: * Sitting for extended periods of time * Typing/data entry for extended periods of time * Lifting/carrying up to thirty (30) pounds * Bending/stooping/climbing stairs All the above duties and responsibilities are considered essential job functions subject to reasonable accommodation.
