Role Responsibilities:Care CoordinationManage day to day operations of Care Coordination office and activities at assigned Affordable Housing site.Provides referrals to service agencies and service providers to the residents and monitor service delivery. Coordinates bi-annual building assessments to identify residents who are at risk. Develop annual service plans.Maintain updated service directory of local resources, create, and maintain partnerships with external partners. Assist in coordinating resident experiences (monthly building meetings, monthly resident newsletters, clubs, events, outreach activities.)Team Leadership Directly supervise assigned Care Coordinators at other designated Affordable Housing sites.
