Families in Transition is currently looking for a The Program Manager- Housing Case Management to join our team!The Program Manager- Housing Case Management will oversee a team of case managers dedicated to supporting individuals and families in achieving and maintaining stable housing. This role is pivotal in ensuring high-quality service delivery, program development, and compliance with relevant regulations. The ideal candidate will possess strong leadership skills, a deep understanding of housing issues, and a commitment to client advocacy. The Program Manager - Housing Case Management is responsible for providing direct support to case managers in delivering wraparound services and prevention assistance to participants facing multiple barriers to self-sufficiency, including physical and behavioral health challenges, domestic violence, and lack of employment or education skills. This role also involves helping clients adapt to community living and programming policies. Essential Functions:
- Staff Management: Hire, train, and supervise housing case managers. Regularly assess competencies through supervision, team meetings, and training sessions.
- Program Development: Develop, implement, and evaluate program policies and practices.
- Case Management Service Monitoring: Oversee the provision of in-home case management services, ensuring adherence to a strengths-based model that utilizes harm reduction, trauma-informed care, and evidence-based practices. Assess accessibility to services and the implementation of Housing First Model for housing retention. Assist participants with navigating Property Management and Housing Authority needs/concerns to foster self-sufficiency towards longer term housing stability (lease violations, demands for rent, income adjustments, recertifications, etc). Model empathy, cultural competency, and person-first language in service delivery.
- Crisis Intervention: Support crisis intervention, conflict resolution, de-escalation, and psycho-education services for participants whose conditions pose risks to themselves or others.
- Community Engagement: Build and maintain relationships with community partners and volunteers to enhance access to services, housing, and social support. Oversee the use of Screening, Brief Intervention, and Referral to Treatment for substance use disorder referrals.
- Documentation Oversight: Ensure thorough participant file documentation and conduct quarterly audits with the Quality Assurance Team.
- Reporting: Complete professional documentation and reports in accordance with agency policies. Ensure compliance with contract reporting, data collection, and record-keeping through regular file reviews.
- Meetings and Collaboration: Attend internal and external meetings and training as required. Provide on-site support and collaboration with staff to ensure program efficacy and effectiveness. Work collaboratively with the Property Management Team.
- Program Activities: Direct the implementation of activities, groups, or workshops focused on community building, skill development, and participant empowerment. Support the identification of incentives for participant engagement and collaborate with other agency departments.
- Reporting: Report on measurable outcomes and analyzing data to make recommendations for program growth.
- Training Participation: Participate in in-service training and professional development.

- Education : Master's degree in related field, preferred. Bachelor's degree (B. A.) from four-year College or university in related field, required.
- Knowledge and Experience with Microsoft 365 Office products; Outlook, Word, Share Point and Excel.
Preferred Qualifications:
- Previous supervisory Experience; 2-3 years
- Five (5) years' experience providing case management or related services to individuals experiencing homelessness or otherwise vulnerable populations.
