Job DetailsJob Location Leimert Park - 4401 - Los Angeles, CA Education Level MSW Salary Range $88,243.00 - $99,150.00 Salary Job Category Nonprofit - Social Services ROLE SUMMARY: The Integrated Services Model (ISM) Program Manager acts as a primary liaison between the Director and ISM staff, and ensures the efficient day-to-day operations of the program at White Memorial Hospital. ESSENTIAL FUNCTIONS:
- Responsible for the overall management and implementation of new ideas and improvements of the ISM program. This includes oversight of outreach, Peer-based Enhanced Services, and Linkage and Advocacy (PESLA).
- Coordinates the provision of wellness services to improve the community's understanding of the connection between physical and mental well-being and to improve access to culturally sensitive integrated mental health, physical health, and substance abuse treatment options.
- Works collaboratively with the Director and Assistant Director of Outpatient Services at Leimert Park to manage day-today operations to ensure program and contract compliance.
- Supports the development of program policies, procedures and workflows.
- Directly supports the recruitment, selection, training, supervision, and evaluation of direct service staff.
- Communicates with Department of Mental Health and program partners for purposes of collaboration regarding referrals for the program as well as attending partner and community meetings.
- Coordinates regular team meetings with a multidisciplinary team at Wellnest to review their existing caseload, discusses new and ongoing clients, and provides feedback about client activities in the recovery process.
- Identifies opportunities and coordinates participation in community health fairs/community events to create access and linkage to ISM services.
- Builds partnerships with community resources including non-traditional resources like Zumba classes to promote ISM services and promote exchange of information and resources.
- Assists in the development of culturally competent educational/outreach material, such as flyers and brochures to reduce stigma and to provide guidance for those seeking both general information on health-related topics and further support in receiving healthcare services.
- Identifies, recruits, screens and enrolls individuals/families from the underrepresented ethnic population community into the ISM program, including individuals/families that have a history of dropping out of services.
- Identifies and develops initiatives to promote patient satisfaction and advocacy efforts.
- Assists in development of tracking and evaluation system to assess the impact of the outreach staff on patient referrals to the clinic.
- Completes other duties, as assigned.
REQUIRED QUALIFICATIONS: Minimum of a master's degree or equivalent (MSW, MA, Ph.D., PsyD, or MS) plus clinical experience and/or training directly related to adolescent counseling required. Bilingual Spanish speaking ability is required. Current registration with licensing board is required and at least two (2) years post-licensure experience in supervising is preferred. A minimum of two (2) years of administrative experience in implementing and supporting programs is preferred. - Annualized Salary Range: $88,243 to $99,150
