Overview:
The Program Manager of the Healthy Families Program will report directly to the Program Administrator. The Program Manager will oversee the day-to-day operation of the Healthy Families grant-funded program ensuring compliance with HFNY, HFA and federal standards and guidelines.
Responsibilities:- Establish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employees
- Contributes to and participates in the Performance/Quality Improvement activities of the assigned department. Contribution and participation includes data collection, analysis, implementation of and compliance with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the BronxCare Health Systems (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service.
- Oversees the day-to-day operation of the Healthy Families grant-funded program.
- Will provide support on a part-time basis to the Program Supervisors and other program staff as needed, this includes Family Support Workers and Family Assessment Workers to ensure the program is in accordance with HFNY guidelines, HFA and federal standards and guidelines.
- Will with the Program Administrator recruit, hire and train program staff.
- Will do random audits to ensure that the data system is updated regularly and in a timely manner in accordance with HFNY guidelines, HFA and federal standards and guidelines.
- Will work with the Program Administrator to develop specific plans for the implementation of program activities.
- Provides support to Program Administrator in regards to policy updates and/or modifications and ensures timely integration of those policy changes into the program. Updates Policy and procedures manual as necessary.
- Conducts Quality assurance activities i.e. chart reviews, supervision documentation, assessment documentation, monitor census, monitors home visiting rates, ensures the worker observations are completed on schedule and other activities related to QI. Develop responsive interventions to address any needs identified in the quality assurance activities.
- Provides support to Program Administrator, helps identify community resources for families served.
- Will interface with community providers to build and renew linkage agreements. Conducts bi annual meeting with Community partners to help identify accomplishments and challenges.

Qualifications:- Must have at least 4 years working with high-risk families in an underserved community and at least 2 years managing staff is required, preferably in an home visiting program.
- Masters Level Degree in Social Work/ or Health
- Must be able to use MS Word, PowerPoint, Excel
