Program Manager – Navigation and Patient Assistance for Duke Cancer Institute

Position is based in Durham NC, hybrid with minimum 3 days onsite.

General Description:

The Program Manager of Navigation and Patient Assistance is a vital role within the Duke Cancer Institute to ensure the quality and consistency of our Navigation workflows for patients in addition to standard processes for Patient Assistance. The Manager has oversight of the Navigator team across all Duke Cancer Institute locations, and is responsible for providing them with standard education, training, and support. The Manager is responsible for continually evaluating staffing and resource needs and partnering with disease group leaders to expand Navigation services for patient populations with high-risk needs and barriers. The Manager will also have oversight of the Patient Assistance program working with Patient Navigators and Social Workers across all Duke Cancer Institute locations and ensure alignment to Duke Health policies.

Duties and Responsibilities of this Level

Continually works to update and maintain the Navigator workflows to ensure team is working to top of license and providing the best patient quality and experience. Partners with other Care Team members to clarify and identify roles and responsibilities as needed.

Coordinates planning cycle and works with Cancer Support Services Administrative Director and other Duke Cancer Institute (DCI) leaders on the creation and implementation of annual strategic goals and priorities for DCI Navigation across all Cancer locations. Continually evaluates program objectives and goals and implements changes as appropriate. Presents to leadership as needed about the status of the DCI Navigation Program.

Directly coordinates and maintains communication with other disciplines across the Duke Cancer Center Care Team including physicians, nurses, social workers, supportive resources, etc. Continually works to improve referral pathways and to improve navigation services for all patients. Help support and advocate for additional navigation resources and position requests.

Coordinates and champions the integration of patient navigation across the DCI, including standard processes and data collection.

Leads team and DCI-wide meetings to ensure compliance with established policies and to keep team aware of current events and updates. Follows up on action items from the meetings.

Reviews program metrics and performance outcomes to identify opportunities and future projects.

Works to develop consistent onboarding, clinical education and professional development for all Navigators, in partnership with Nurse Navigator Team Lead. Leads efforts to continually re-educate Navigators on Duke and community resources available.

Continually shares best practices from other organizations and works to improve DCI patient navigation.

Meets 1:1 with direct reports and provides case supervision and case consultation. Available for questions and to help troubleshoot issues.

Promotes an atmosphere of open communication that facilitates team member input into decision-making, resolution of conflict, and collegiality among all team members. Is present and available for team members onsite at all Duke Cancer Institute locations.

Administers various personnel actions including, but not limited to, position request creation, recruitment and hiring, onboarding, performance reviews, timecard approval, disciplinary actions and other related activities. Compile and maintain records, reports and documentation of program to be used for program evaluation.

Manages Patient Assistance program budget for Lodging, Transportation and emergent Financial Assistance grants; regularly researches and seeks out new funding sources to share with team; submits applications for grant funding; reviews emergent Financial Assistance grant applications for approval.

Regularly communicates Patient Assistance budget and actual expenses to Patient Navigators and Social Workers; ensures alignment to Duke Health policies

Other projects and duties as directed.

Education

Work requires communications, analytical and organizational skills generally acquired through completion of a bachelor's degree program.

Experience

Work requires the ability to plan and administer programs and direct program activities within a specific functional area, generally acquired through four years of related experience.

Patient Navigation experience Preferred

OR ANY OTHER EQUIVALENT COMBINATION OF RELEVANT EDUCATION AND/OR EXPERIENCE

Degrees, Licensure, and/or Certification

RN License Preferred
