Job DescriptionProgram Manager - Parker Care Navigator - Pavilion - Exempt - Eligible for benefits - 9:00am - 5:00pm, Monday - Friday - The Program Manager-Parker Care Navigator will provide service coordination for older adults living at home in Middlesex and Somerset Counties. Parker Navigator provides professional guidance to help clients and their families to live their best lives at home. Responsible for outreach to the community informing and educating local clinicians, community groups, advocacy and senior organizations about Parker Navigator services and benefits. The Navigator will engage clients and provide service/resource coordination so clients are empowered through education and resource sharing to promote and prolong their independence at home.   • Master's degree in Social Work (MSW), Gerontology, or related degree preferred OR be a Registered Nurse (RN) with experience working with the older adult population and/or their families
  • Minimum two (2) years of geriatric experience required
  • Aging Life Care Professional membership preferred
  • New Jersey Driver's License in good standing
  • Demonstrated critical thinking and problem-solving skills
  • Ability to work with minimal supervision
  • Field work and Home/Site visits required
  • Proficiency in Microsoft Applications
  • Flu vaccine required, or receive medical exemption prior to employment
  • Covid-19 vaccination documentation will be requested for reporting purposes
