Job Description:
This is a highly responsible position within OBH's Division of Health Plan Management (HPM). This position functions under the direct supervision of the Program Manager 1B over the My Choice Louisiana Transition Coordination in the Community Integration/Best Practices section of HPM. The incumbent employee in this position acts as principle staff to the transition of individuals from NF into the community, also supporting individuals transitioning from state-run inpatient psychiatric facilities when called upon to do so. This includes collaboration with Pre-Admission Screening and Resident Review (PASRR) program staff, transition coordinators employed through the Office of Aging and Adult Services (OAAS), Managed Care Organizations (MCOs), state-run psychiatric facilities and other community partners. The incumbent in this position will be responsible for the coordination, facilitation, and monitoring of transitions into the community in compliance with established guidelines. As needed, this responsibility extends to directly assisting individuals, organizations, and providers in the process of transition (direct service). Other responsibilities can include providing training, disseminating information, coordinating regional and statewide information/resources listings, providing technical assistance, and facilitating resolution of individual transition issues. The incumbent will participate in policy development and planning at the state office level in order to complete longitudinal planning to maintain or enhance community capacity to support demonstration transition targets. This will include combining state and local initiatives and resources to support the transition of individuals from nursing facilities. This may also include participation in policy development and planning that involves multiple administrative units within the Louisiana Department of Health and across multiple state agencies.
ESSENTIAL FUNCTIONS
50% Responsibilities include facilitating and monitoring nursing facility transitions in compliance with developed standards for operation. This may include, but is not limited to:
Outreach to NF residents, inquiring about their interest in transitioning into the community
Conducting, or facilitating the completion of, screening and evaluations intended to identify need upon discharge into the community
Obtaining pertinent documentation from nursing facility records while in facilities
Coordinating with NF staff, OBH, OAAS transition staff, and MCOs regarding the provision of services
Providing ongoing communication with the residents and their supports, ensuring a person centered plan for discharge is identified.
Work with community housing providers regarding transition
Assist in the completion of all applications for public assistance, SSI/SSDI, and housing, intended to support the individual upon transition to the community
Monitoring/updating spreadsheets for accuracy and documentation of progress in transition
Facilitation of scheduled conference calls with MCOs, providers, OAAS and OBH staff, as well as any other relevant entity to assess and monitor transitions in process and participants who have transitioned
Communication, support and training of Office of State Ombudsman (OSO) staff, and others as needed
Resource development
Identification and communication of changes/revisions needed in current policies related to nursing transitions

Maintain program and individual-level data on transition processes; identify trends and patterns on a local and statewide level. Develop suggestions for response to monitoring findings, programmatic challenges and issues.
25% Monitor the status of transitions against individual data indicators and transition targets. Report these findings to OBH staff. Provide necessary support and collaboration to partners engaged in the discharge process to ensure successful transitions. These partners includes OBH, MCOs, behavioral health providers, OAAS transition staff and support coordinators, Office of State Ombudsman staff, nursing facility staff, the resident and their support network. This process of support and collaboration includes providing education, expertise, resource referrals, leadership when working with the above mentioned transition team.
Experience Preferred:
1) Experience working with individuals with behavioral health conditions and complex medical needs
2) Experience with one or more of the following:
-Transition planning from nursing facilities; support coordination/case management, social services coordination
- Medicaid or program office functions on the state or regional level; knowledge of the Medicaid and MCO system of care; experience with person-centered planning.

Minimum Qualifications
MINIMUM QUALIFICATIONS:
A baccalaureate degree plus two years of professional level experience in administrative services, economics, public health, public relations, social services, health services, or surveying and/or assessing health or social service programs or facilities for compliance with state and federal regulations.
SUBSTITUTIONS:
Six years of full-time work experience in any field may be substituted for the required baccalaureate degree.
Candidates without a baccalaureate degree may combine work experience and college credit to substitute for the baccalaureate degree as follows:
A maximum of 120 semester hours may be combined with experience to substitute for the baccalaureate degree.
30 to 59 semester hours credit will substitute for one year of experience towards the baccalaureate degree.
60 to 89 semester hours credit will substitute for two years of experience towards the baccalaureate degree.
90 to 119 semester hours credit will substitute for three years of experience towards the baccalaureate degree.
120 or more semester hours credit will substitute for four years of experience towards the baccalaureate degree.
College credit earned without obtaining a baccalaureate degree may be substituted for a maximum of four years full-time work experience towards the baccalaureate degree. Candidates with 120 or more semester hours of credit, but without a degree, must also have at least two years of full-time work experience to substitute for the baccalaureate degree.
Graduate training with eighteen semester hours in one or any combination of the following fields will substitute for a maximum of one year of the required experience on the basis of thirty semester hours for one year of experience: public health; counseling; social work; psychology; rehabilitation services; statistics; experimental/applied statistics; economics; business, public, or health administration.
A Juris Doctorate will substitute for one year of the required experience.

Graduate training with less than a Ph.D. will substitute for a maximum of one year of the required experience.
