The Supervisor ensures the program maintains regular Medicaid compliance, managing month to month billing processes, and use of health information technology to inform practice. The Program Supervisor will be responsible, together with the Director for Care Coordination and Health Homes, to guide the program towards achieving Health Home goals. This position has significant supervisory, administrative and program management responsibilities. The Program Supervisor is responsible for ensuring their team functions optimally by meeting contractual obligations, and contributing to a culture that is client centered, outcome oriented, and dedicated to continuous quality improvement. ESSENTIAL FUNCTIONS: Responsibilities include but are not limited to the following:
- provides clinical and administrative supervision to Care Coordinators to monitor performance, provides training to teach new skills and coaches (both in the field and in the office) to improve performance;
- staff recruitment and hiring;
- assesses the skill level of staff and develops individualized training plans as well as recommends the appropriate internal/external workshops and trainings to increase staff development;
- tracks staff work activities including outreach, intake, assessment, service planning and delivery, referrals and linkages to community-based organizations, follow-up, collaboration with collateral contacts, documentation, confidentiality and contact standards;
- develops and implements performance improvement plans and manages progressive disciplinary process as needed;
- writes and delivers performance appraisals;
- conducts quality assurance reviews to assess the quality and effectiveness of services delivered, as well as ensures member charts and documentation are in compliance with Health Home agreements;
- develops and implements action plans to improve effectiveness and efficiency of staff;
- oversees the care provided by the care management team, through case conferences, field visits, and customer service calls;
- responds to inpatient and ER admission alerts and contacts medical provider/hospitals for admission/discharge information;
- assists in the planning and implementation of operations procedures and provides program management with continuous feedback about operations;
- serves as the point of contact for crisis intervention services;
- maintains 24/7 availability for staff and client emergencies;
- ensures staff maintain clinical documentation and records that uphold HIPAA regulations;
- utilizes multiple Electronic Health Record systems;
- maintains client assignments and maintenance of rosters;
- reviews all billable contact notes and ensures monthly billing is submitted to the Office Manager by end of day the first business day of the month;
- responsible for weekly, monthly and quarterly reporting of program information to the Program Director and Assistant Director;
- develops, delivers and participates in ongoing professional trainings;
- represents PCMH in local and state meetings as required;
- other duties as assigned by the Program Director and/or Executive Management.
QualificationsKNOWLEDGE:
- Knowledge of Health Home Care Coordination
- Knowledge of Medicaid, Social Security and other entitlements.
- Knowledge of community resources available to the population served.
- Computer literacy; including Office programs and EHRs.
SKILLS AND ABILITIES:
- Excellent oral and written communication skills.
- Leadership ability.
- Ability to multitask.
- Well organized with attention to detail.
- Strong time management skills. Able to work effectively to manage workload under pressure and meet deadlines.
- Ability to work with multiple electronic health records.
EDUCATION AND EXPERIENCE:
- Licensed Mental Health Professional (LMHC, LMSW, LCSW)
- Excellent communication skills, ability to multi-task and prioritize work, and be able to supervise staff in a culture of accountability.
- 4 years of experience working with the seriously mentally ill to supervise the provision of services to Medicaid recipients with chronic medical and mental health issues. Must be knowledgeable about Health Home regulations and comfortable working with clients in the field.
