Responsibilities
**Position Summary:**
FNP assumes direct patient care, coordination of clinical services, assistance with team functions, and assurance of continuity of care.
**Qualifications:**
Direct Patient Care
+ NP resident provides direct patient care to patients throughout the lifespan during precepted continuity clinics. Provides care according to professional Nurse Practitioner and community standards for advanced clinical practice utilizing clinical practice and Family Medicine guidelines, protocols, and procedures. Patient care guided by state and federal laws, DEA regulations, and the New York State Nurse Practitioner Modernization Act.
+ Examines patients and prescribes treatment when indicated for individuals and families requiring acute and chronic health care, preventive and restorative care, and evaluation and care of physical, mental, and developmental function and capabilities.
+ Recommends and refers patients for additional treatment or evaluation as necessary.
+ Completes all required EMR documentation related to patient care.
+ Counsels and educates patients, their families, and their care givers on self or directed care intended to maintain or restore physical or emotional health or function.
+ Counsels and educates parents and other direct care providers of minor children and adults, including the elderly, regarding special care needs, emphasizing normal growth and development, positive parenting, maintenance of independence, safety, appropriate use of medications and support services, nutrition, restoration of health and function, and illness and disability prevention. As part of the Patient Centered Medical Home may be the continuing care provider for his or her own patient panel.
+ Provides phone care to patients and serves as a resource person to the team nurse regarding phone care, triage evaluations for acute care, and patient management when clinical decisions are required. In addition, participate in the on-call schedule (with back-up) during off-hours per practice protocols.
+ Required off-sessions (nights and weekends).
Education
+ Participates in structured post-graduate residency training sessions to include rotations through specialty clinics, weekly formal learning sessions on a variety of complex clinical challenges, chronic disease management, interprofessional projects focused on population health, didactic training in leadership, and participation in team PDSA cycles to improve health system processes.
+ Assists in the development of an interprofessional collaborative practice, including but not limited to role modeling shared care, equality in decision-making, and professional accountability.
+ Serves as a resource to the medical assistants and RN staff for patient care and clinical questions.
Administration
+ Completes all outside agency forms and ensures completion of all EMR requirements surrounding direct patient care.
Liaison/Team-Based Care
+ In collaboration with the RN team, reviews lab reports to assure safety and continuity of care of team patients.
+ Monitors patient safety and intervenes appropriately to assure safe patient care.
+ In collaboration with the primary care team, maintains communication with and coordinates care provided by outside agencies.
+ Works with the healthcare team to assess patient risk. Develops patient care plans and coordinates care in community settings across the regional integrated delivery system.
+ Delivers off-site patient care as necessary. This may include care in the home, hospital, extended care, or other facility.
Professional
+ Meets regularly with NP residency Director. Attends in-service meetings, grand rounds, M&M case presentations, QI Committee, and participates in peer review and intraprofessional team-based trainings.
+ Maintains clinical competence and knowledge proficiency. Attends continuing education and professional development programs as required by certifying and credentialing organizations.
Additional Duties and Responsibilities:
May conduct independent or collaborative research per approval of NP residency Director.
+ May write articles for publication in professional journals, make presentations to continuing education, community, and professional programs, participate in professional nurse practitioner organizations to foster the development of the nurse practitioner role per approval of NP residency Director.
+ Assists team nurses with patient care functions as needed.
+ Adjusts direct care interventions to meet patient needs across the lifespan.
+ May contribute to the development of patient education materials used at HFM.
+ Incorporates the URMC ICARE values into daily functions.
**Qualifications:**
Education: Fulfills the educational requirements to be a licensed FNP in NYS.
Experience: Prefer 3-5 years of experience, as a registered nurse prior to completing a Master's program is desired.
License/Certification Required: Meets the requirement to be a licensed nurse practitioner in the State of New York for specialty practice as a Family Nurse Practitioner with full prescriptive privileges. Active registration by the Federal Drug Enforcement Agency to prescribe Category II through V drugs. National certification in Family Practice preferred from a national organization, such as, but not limited to, the American Nurses Credentialing Center of the American Nurses Association or the American Academy of Nurse Practitioners.

**Pay Range**
Pay Range: $67,267 - $94,141 Annually
_The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations._
