About the Position The Project Supervisor position assists and aids individuals referred through the Supportive Health Services (SHS) Programs and provides leadership within the Northern California SHS Region. This position will also provide intensive case management services for special needs populations such as chronically homeless, Veterans, and/or individuals with physical and/or behavioral health challenges. Core Duties:

- Follow all LifeSTEPS policies, procedures and guidelines.
- Empower staff to manage competing priorities, projects and all aspects of intensive case management service delivery.
- Promote, coordinate and tailor services to meet identified needs.
- Document all client contacts as necessary using multiple database systems.
- Complete all administrative assignments including timely data entry in multiple databases.
- Review service plans and perform follow ups to determine quantity and quality of services provided participants and status of participant cases.
- Partner with Regional Director of Supportive and Health Services in training new team members for stronger starts. Recruit and onboard new staff and provide ongoing support and training to all staff.
- Conduct site visits and model case management best practices.
- Provide Intensive Case Management coverage as needed during recruitment and other staff absences.
- Carry out supervisory responsibilities in accordance with the organization's policies and applicable laws, specified grant requirements, and specified contract/program requirements.
- Review case records. Compile and review content of myLifeSTEPS.org and HMIS database entries, and any other required reports. Ensure they are accurate and turned in by applicable due dates.
- Assist with Paycom duties to include timecard and expense approval and processing of new hires, terminations and change PAF's.
- Provide direct supervision and oversight to assigned staff.
- Conduct ongoing weekly supervision and training calls with assigned staff.
- Create and deliver training opportunities for staff
- Maintain staff records as required by program
- Submit regional impact stories and requests for activity pictures each month as assigned.
- Assist in the processing of the Client Assistance Funds.
- Prepare, distribute, and maintain a variety of reports.
- May need to work some evenings and weekends in order to attend events or address situations of crisis.
- Regional travel and occasional overnight travel may be necessary.
- Other duties and responsibilities as assigned.
Intensive Case Manager Duties
- Assess participants to identify needs and access to services needed to address immediate basic needs.
- Maintain regular and ongoing client contact; meet bi weekly (at a minimum); conduct monthly unit safety inspections.
- Ensure clients are linked to and accessing medical, mental health and substance use disorder services as needed.
- Assist clients with:

- maintaining medication and treatment regimens.
- maintaining income and/or establishing public benefits.
- locating and securing employment, volunteer and/or educational opportunities.
- gaining, restoring, improving or maintaining daily independent living, social, leisure and personal hygiene skills.
- budgeting and money management.
- monitoring any legal issues and overcoming any financial situation that might prevent them from accessing and maintaining permanent housing and supportive services.
- Educate clients on the appropriate use of crisis intervention services instead of using 911 emergency calls for assistance.
- Educate clients on tenant rights and responsibilities, including compliance with lease policies, paying rent on time and how to budget finances.
- Attend scheduled medical or mental health appointments with clients to assist with successful attendance and completion of such appointments.
- Work closely with client, property management staff and other partners to help clients resolve issues that may threaten housing stability.
- Maintain weekly contact with the property manager, and communicate information on property as appropriate
- Maintain appropriate client case files to include documentation of all client contacts and contacts to service providers made on behalf of the client; clearly document all eviction prevention interventions provided to client; prepare reports as directed.
- Conduct client outreach as required to complete initial client contact, including outreach to local agencies or service providers.
- Create client service plan and assist client in identifying goals and completing those goals.
- Review client service plan and assist client in identifying progress towards goals, assessing if goals are still relevant, and assisting client in completing those goals.
- Research, review and access local community resources and assist clients in referral follow up.
- Conduct and/or facilitate community building activities to promote integration for all residents and clients at project-based sites.
- If LifeSTEPS is also the resident services provider, collaborate and coordinate services with Director of Social Services when appropriate.
- Make reports to Child Protective Services or Adult Protective Services as mandated reporters.
- Provide case management services to assigned resident communities that are in alignment with the LifeSTEPS mission, vision and values.
QualificationsEducationand/orExperience: The minimum qualifications and training for this position are:
- Bachelor's degree in social work, psychology or related field.
- Two (2) years of experience in a similar environment providing social services.
- Minimum of (1) year experience as a lead or ICMS supervisor, or demonstrated experience in a supervisory role.
- An equivalent combination of education and experience may substitute for the degree.
- Experience in providing Intensive Case Management to homeless and formerly homeless populations
- Strong administrative skills required, including experience with database entry
