Job Summary and QualificationsResponsible for coordinating all patient information to be submitted to CMS for Medicare reimbursement and maximizing LOS efficiency in timely and accurate completion. Coordinate and monitor CMS compliance. This individual provides clinical support in completion of data contained in the IRF-PAI to ensure accuracy in the scoring of functional status and reflection of the patient's care needs. In addition, this position assists in the ongoing monitoring of identified benchmarks for the inpatient rehab population. The PPS Coordinator will have access to confidential patient information and maintains confidentiality of all information.
1. Oversees and ensures the accuracy and completion of the Inpatient Rehabilitation Facility – Patient Assessment Instrument (IFR-PAI) prior to electronic transmission within requires time frames
2. Maintains a current knowledge of IRF-PAI software and provides expertise on this software within the inpatient rehab setting
3. Maintains updated and ongoing knowledge of the regulations concerning the IRF-PAI and its completion within the inpatient rehab setting
4. Thoroughly researches and reviews clinical documentation and makes recommendations to clinical supervisors in areas needing improvement to support and validate rehab criteria and documentation compliance
5. Collaborates with physicians and interdisciplinary team members to capture accurate assessment of patient, including interdisciplinary patient treatment plans to facilitate achievement of expected outcomes in collaboration with interdisciplinary team
6. Verifies appropriate CMG (case mix group) designation through chart review, staff interview, and collaboration with the rehab medical director
7. Participates in ongoing education to the staff to ensure accurate and timely completion of the IRF-PAI
8. Periodically audits IRF-PAI, clinical documentation and billing information to ensure accuracy and consistency of documentation. Works closely and collaboratively with all members of the clinical team to ensure compliance at all levels
9. Evaluates patterns of variances from standardized care plans and recommends strategies to resolve individual and caseload issues
10. Serves as a resource to department staff, students, volunteers and other departments, and contributes to the teamwork essential to ensuring quality patient care
11. Promotes participation, cooperation and teamwork. Demonstrates problem solving in resolving issues
12. Participates in hospital or departmental performance improvement activities and seeks opportunities to improve department and inter-department processes
13. Participates in internal chart review and external audit coordination as needed
What qualifications you will need:Bachelor's degree from an accredited school in Nursing or Physical Therapy
Prefer at least 2 years previous experience as a licensed clinician in an Inpatient Rehabilitation and a working knowledge of IRF-PAI & CARE Tool
