GENERAL PURPOSE OF THE POSITION:

The Psychiatric Rehabilitation Coordinator is responsible for implementing Brighter, Stronger Foundation's participant rehabilitation plan, and working with Brighter, Stronger Foundation participants and their families to provide appropriate services. The Psychiatric Rehabilitation Coordinator will work with Brighter, Stronger Foundation participant(s) in the community and their homes or onsite.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Client Services Implementation and Management

  • Responsible for ensuring assigned caseload is reaching a higher level of independence and integration within their community
  • Operationalize the mission, vision, and core values of Brighter, Stronger Foundation.
  • Responsible for assessing and connecting assigned caseload in accessing and coordinating benefits, services and community-based resources
  • Maintain consistent client services for assigned caseload, in accordance with assigned schedule
  • Document and address the rehabilitation needs of the client including: client's strengths, skills, and needs; current resources and support systems; as relevant, legal status; individual substance abuse history if any; educational history; home environment
  • Implement age-appropriate rehabilitation goals and activities outlined in the individual's IRP.
  • Work cooperatively with family and treatment team members to coordinate services with other agencies and programs in the best interest of the individual.
  • Collaborate with assigned caseload's mental health provider, at a minimum monthly
  • Ensure client safety and engagement throughout the provision of services
  • Complete regular review and updates of rehabilitation goals outlined in the individual's IRP, which involve recovery and restoration of skills.
  • Communicate client and programmatic needs with Supervisor.
  • Ensure client routines and schedules are carried out promptly and consistently and that they are developmentally appropriate.
  • Properly report all critical incidents and safety concerns regarding persons receiving service
  • Collect data on all targeted goals and meet with the supervisor to analyze the information gathered and participate in initial and ongoing training.
  • Model and promote effective communication, customer service, and positive representation of the organization to clients, customers, referral sources and the public.
  • Effectively use Brighter, Stronger Foundation behavior guidance techniques to help clients and families decrease maladaptive behaviors.
  • Works cooperatively with other BSF professionals to support assigned caseload
  • Maintain consistent communication with parents, supervisors and coworkers on client goals and progress.

Administrative

  • Record timely and complete documentation of services in the EMR per policy and organizational procedures.
  • Record time worked, and scheduled/unscheduled time off, in the HRIS per policy and organizational procedures.
  • Individual Performance Management:
  • Attend departmental meetings and supervision meetings as scheduled.
  • Complete annual self-evaluations in accordance with the required timeline.
  • Complete all required training during the first 60 days of employment, first year, and annually.
  • Maintain a reliable attendance record.

OTHER DUTIES AND RESPONSIBILITIES:

Other projects/duties as assigned for the overall benefit of the organization

Requirements:

EDUCATION AND EXPERIENCE AND NECESSARY KNOWLEDGE, SKILLS, AND ABILITIES:

  • Minimum of one (1) year Experience working with youth and/or adults with emotional disorders or severe and persistent mental illness.
  • Be under the direction of a rehabilitation specialist who is: A licensed mental health professional; Certified by the Commission on Rehabilitation Counselor Certification; or certified by the Psychiatric Rehabilitation Association.

Education and/or Experience:

  • High School Diploma or GED from an accredited school required, associate or bachelor's degree in psychology, social work, or a related field preferred.
  • Clean driving record with no more than two (2) points.
  • Must have a satisfactory CJIS report.

Compensation details: 21-28 Hourly Wage
