The primary responsibilities include conducting initial psychiatric evaluations for diagnosis and medication management, participating in treatment planning, and managing ongoing medication needs for established clients. Additional duties involve ordering lab tests and toxicology screenings, providing medication education, consulting and collaborating with other CCBHC staff through case consultations or team meetings, and documenting in the EHR system, including E-prescribing. As part of the CCBHC team, you must be willing to work collaboratively with both internal staff and external providers to promote integrated care. This position requires on-site work in the office.ESSENTIAL FUNCTIONS:Conduct initial psychiatric assessments, diagnosis and updates as clinically indicated.Provide ongoing medication management through E-prescribing.Educate individuals and families about the medications and their effects.Coordinate consumer access to medications with the RN and LPN including administering injectable medications when needed.Order routine blood work and/or toxicology testing as needed.Document all services and outreach efforts via progress notes in the EHR within 24hours.Participate in the development of person-centered treatment plans and sign to coordinate and authorize treatment.Consult with team to discuss individual cases and coordinate care.Provide medical consultation to a multi-disciplinary CCBHC staff.QualificationsSKILLS AND KNOWLEDGE:Must have a commitment to understanding patients in the context of their socio-cultural experience.Leadership ability in providing psychiatric and substance use treatment to a diverse population of children and adult clients/families and providing psychiatric consultation to multi-discipline staff.Excellent communication skills and ability to interact with all clinical and administrative staff, other professional staff and agencies, clients, and families.Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred.EDUCATION AND EXPERIENCE:NYS License in Psychiatric Mental Health Nurse Practitioner.Experience working with children.Experience with addiction medicine. Experience with psychiatric evaluations, psychopharmacology, MATs.Extensive experience working with people with serious mental illness, co-occurring substance use disorder and primary health care concerns.
