We are seeking a compassionate and dedicated Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our client's outpatient behavioral health team. This hybrid role involves providing high-quality mental health care at our outpatient center and conducting home visits for patients two days a week. This is an excellent opportunity to make a meaningful impact on patients' lives by delivering personalized, patient-centered care.*Key Responsibilities:** Perform psychiatric evaluations, diagnose mental health conditions, and develop individualized treatment plans.* Prescribe and manage psychotropic medications, providing ongoing medication management and monitoring.* Provide psychotherapy or counseling to patients as appropriate.* Visit patients in their homes two days a week to deliver care in a familiar and comfortable environment.* Assess patient needs, support care plan adherence, and provide education to patients and families.* Work closely with an interdisciplinary team of therapists, case managers, and other healthcare professionals to coordinate comprehensive care.* Communicate effectively with patients, families, and community resources to ensure optimal outcomes.* Maintain accurate and timely documentation of patient care in electronic health records (EHR).* Ensure compliance with regulatory standards and organizational policies.*Requirements:** Master's or Doctorate degree in Nursing with specialization as a Psychiatric Mental Health Nurse Practitioner.* Active and unrestricted PMHNP license in NY.* Minimum of 2 years of experience in a psychiatric or mental health setting preferred.* Experience with home-based care is a plus.* Strong diagnostic and clinical skills in mental health care.* Excellent communication and interpersonal abilities to work with diverse populations._Please note- The annual base salary range is $130k to $150k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things.
