Job Title: Psychiatric Nurse Community Program (Taunton, MA) Program Name: AT-PACT / B-PACT Position Type: Full Time/Part time Qualifications: Bachelor's degree in Nursing Experience in psychiatric nursing or relevant specialty Key Responsibilities: Assess the overall physical health of clients Conduct health assessments and ongoing evaluations Collaborate with clients to create and implement personalized treatment plans Lead the coordination and delivery of physical health treatments Build relationships with community healthcare providers Accompany clients to medical appointments and ensure follow-up General Responsibilities: Participate in ongoing assessments and support for PACT clients Provide direct support, including emotional and practical assistance Teach daily living skills, such as grocery shopping and self-care Advocate for clients to access services and benefits Locate and set up support groups for clients and families Attend team meetings to ensure a collaborative approach to care Other Requirements: Valid driver's license and clean driving record Completion of MA and ADP Criminal Offense Record Information (CORI) process Basic computer skills related to job responsibilities Benefits: All Standard Benefits.If you are passionate about providing care in a supportive environment and meet the qualifications, we encourage you to apply!
