Psychiatric Nurse Practitioner, Outpatient. Group offer unique and individualized care to the aging population, those with developmental disabilities and those suffering from extreme trauma. Patients are supported by a team of mental health counselors, social workers, certified nurse practitioners, case managers and psychiatrists to ensure the best outcomes for those we serve. The psychiatric nurse practitioner performs face to face assessments and ongoing psychiatric visits. Maintains current patient records in order to clearly state problems, plans, actions, goals and changes as they occur. Instructs patient caregivers and Manages client medications. Position is split between an outpatient caseload, tele psych visits and facility visits (normally long-term care facilities). The schedule is negotiable offering Monday through Friday 8am 5pm, 4 -10 hour says or part-time hours.
