POSITION SUMMARYThe Psychiatric Rehabilitation Services Coordinator (PRSC) is responsible for coordinating and overseeing the delivery of psychiatric rehabilitation services to groups and individuals with mental health disorders. The Psychiatric Rehabilitation Services Coordinator will work collaboratively with an interdisciplinary team to develop and implement therapeutic services and interventions to ensure that the emotional and social needs of the resident are met and maintained on an individual basis. ESSENTIAL JOB FUNCTIONS
  • Develop, implement, and coordinate psychiatric rehabilitation programs tailored to the unique needs of individuals with mental health disorders.
  • Conduct thorough assessments of residents' psychiatric, social, and vocational needs.
  • Collaborate with residents to establish realistic rehabilitation goals and monitor progress.
  • Provide ongoing case management and support to individuals in the psychiatric rehabilitation program.
  • Collaborate with psychiatrists, therapists, social workers, and other healthcare professionals to ensure a holistic approach to psychiatric rehabilitation.
  • Participate in interdisciplinary team meetings to discuss individual cases and treatment plans.
  • Facilitate Care Plan conferences for each resident in the assigned case load, prioritizing needs, writing goal statements and discharge plans with the interdisciplinary team. Care plans may be in response to significant change, initial for new admissions, quarterly and annual care plan conference.
  • Maintain accurate and up-to-date records, documenting progress and any changes in treatment plans.
  • Make regular rounds during scheduled shifts to ensure the safety of residents and staff.
  • Stay current on facility policies and procedures on behavioral emergencies and be trained in crisis management and diffusing a potentially volatile situation, especially concerning aggressive/agitated behavior.
  • Complete other duties as deemed necessary and appropriate, or as may directed by the department Director or Administrator.

QUALIFICATIONS

  • Bachelor's or Master's degree in social work, psychology, counseling, or a related field.
  • Previous PRSC and/or social services experience preferred.
  • Strong knowledge of mental health disorders, treatment modalities, and rehabilitation strategies.
  • Must be capable of performing the essential functions of the job.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult situations.
  • Must meet health assessment requirements set forth by the policies of this facility and the state, including medical and physical examination, two stage Mantoux skin tests and one-step annually or Xray.
  • Must be able to push, pull, move, and/or lift a minimum of thirty-five pounds.
  • Must be able to sit, stand, and walk for an extended period of time bend, but also be able squat, kneel, and bend frequently.

WORKING ENVIRONMENT

  • May be subject to the exposure of infectious waste, diseases, and conditions.
  • May be exposed to blood, body fluids, or tissues.
  • May be subject to the handling of and exposure to hazardous chemicals including tobacco smoke.
  • May be requested to assist in the evacuation of residents during emergency situations.
