Job DescriptionThe Psychiatric Social Worker willformulate assessments and treatment/discharge plan that will highlight a clinically realistic and sensitive understanding of the patient's needs. Their responsibilities include, but are not limited to:
- Interview patient and/or family to obtain psychosocial information and assess the patient's functional level, relative to safety and/or discharge planning needs.
- Formulate an assessment and treatment/discharge plan that will highlight a clinically realistic, and sensitive understanding of the patient's needs.
- Provide direct social work/discharge planning services including community resource advocacy as well as keep ongoing professional rapport with community agencies as needed. Conducts meaningful communication with physicians and conducts timely follow up for completion of required documentations to facilitate discharges.
- Documents all work activity in the patient's medical chart in and Allscript in accordance with departmental and regulatory guidelines.
- Engage with patient and/or family members with respect to cultural differences and sensitivity to individuality.
- Counsel patients, families, and significant others in the areas of expectation regarding life style changes and functional coping patterns.
RequirementsCandidates must have:
- Master's Degree in Social Work from an accredited school of Social Work. Participation in continuing education activities is expected.
- New York State License to practice as a Social Worker (LCSW or LMSW in NYS)
- Minimum 1 - 2 years' experience of inpatient clinical experience in an acute care, home health setting or managed care organization;Experience with Psychiatry Social Worker is highly preferred.
- Bi-lingual English/Spanish highly preferred
