The part-time Case Manager will be responsible for ensuring the continuity of care for students at the Counseling Center who require external services. The part-time Case Manager will provide clinical and outreach services to students and the Pace community.

Position Duties

* Track referrals to insure continuity of care and follow up. Facilitate referrals for the purpose of transfer of care between service providers.

* Maintain a current database of available service providers.

* Assist students in making use of health care benefits.

* Routine monitoring and management of student treatment compliance (i.e. missed appointments, cancellations, etc.) along with necessary consultation with students.

* Clinical assessment of walk in clients.

* Assist with securing hospitalization for students and related discharge planning, when needed.

* Consultation with families, faculty, administrators, and medical providers regarding student mental health issues and treatment options.

* Clinical assistance with outreach, crisis response, and consultation efforts, as needed.

* Attend department, division, and university committees, and case conferences as needed.

* Perform other such duties as may be assigned from time to time.

Position Qualifications

* Minimum of a master's degree in Social Work, Counseling, or Psychology.

* One (1) year full-time experience in mental health case referral preferred.

* Licensed in NY or within one year of date of employment.

* Knowledgeable about and skilled in psychological and medical issues related to serious mental illness.

* Knowledgeable about and skilled in working with a variety of mental health providers and insurance carriers.

* Excellent oral and written communication skills.

* Knowledgeable about and skilled in psychological and medical issues related to serious mental illness in the college student population.

* Proficient in Microsoft Office.

Work Hours

Wednesday and Friday 9 AM to 5 PM as well as one other agreed upon part time day.
