SUPERVISOR: Priscilla Project ManagerHOURS: Flexible part time hours between8:30-5 M-F as agreed, upon with supervisor, some evenings and weekendsCLASSIFICATION: Part-time/Hourly/Non-ExemptPRIMARY LOCATION: 233 West Ferry (must be able to travel to multiple locations as needed)QUALIFICATIONS:
  • Knowledge of the community, community organizations, and community leaders
  • Bilingual skills, depending on the community and families being served
  • Must complete Community Health Worker, interpreter, certified passenger safety technician, and doula training programs once hired
JOB SUMMARY: The Community Health Worker/Doula will work to improve healthy birth outcomes by empowering socially isolated, at-risk women as they go through the process of pregnancy, labor and delivery. They will do this by evaluating, responding to, and advocating for the needs of prenatal and postpartum clients through maternal health case management services. The person in this position will predominantly work in clients' homes and in the hospital.Essential Job Duties COLLABORATIVE
  • Provide advocacy, support and follow-up to determine if services are received and assist families with health behavior changes.
  • Participate in staff meetings and staff development training to enhance and improve personal knowledge of topics applicable to Job Summary.
DOULA EDUCATIONAL REQUIREMENTS
  • Attend a six (6) week doula training program
  • Attend childbirth class
  • Attend a breastfeeding class
  • Shadow three births
  • Write about your experiences during those births
  • Attend CPR class and maintain CPR certification
  • Attend and remain compliant with mandatory organizational annual training
  • Attend at least 6 CEU sessions per year to maintain doula certification
CLIENT CARE
  • Identify issues and assist clients in accessing maternal health care services
  • Aid and/or provide referrals for clients to obtain other essential support services
  • Provide culturally appropriate prenatal and postpartum health education
  • Assist families to identify needs, provide basic health information, and make appropriate referrals through monthly home visits.
  • Maintain accurate records
  • Promote self-sufficiency in meeting short- and long-term needs and goals with clients
  • Provide home visiting services to approximately 20 -25 clients at any given time.
  • Conduct weekly breastfeeding support groups depending on caseload interest

Individuals who hold positions at Jericho Road Community Health Center should possess the following qualification and capabilities:
  • Passion for serving individuals personal and community development
  • Excellent verbal and written communication skills
  • Experience working with EMR, Medent preferred
  • Project-management experience
  • Strong organizational skills and attention to detail
  • Proficiency in Microsoft Office programs or willingness to learn
  • Creativity and problem-solving capabilities
  • Positive and engaging interpersonal skills
  • Strong self-motivation
  • Ability to travel between sites if necessary to serve multiple locations
  • Experience working with low-income, diverse, multilingual populations highly desired
  • Able to interpret, adapt, and apply guidelines and procedures
  • Must be self-motivated and able to function independently and in teams at all levels
  • Strive to serve various communities and socioeconomic statuses in a culturally competent way
  • Attend required training as deemed mandatory by organization or to continue education in field
  • Demonstrate flexibility as new tasks are added by supervisor or position evolves over time
PHYSICAL REQUIREMENTS: Must be able to sit or stand for long periods of time. Exert up to 50lbs. pushing, pulling, or lifting. Walking and climbing stairs required. Able to talk/hear and communicate with others with or without reasonable accommodation.TRAVEL REQUIREMENTS: Must be able to travel to and between sites while maintaining standards of safety and compliance with motor vehicle traffic laws. This position may require travel for necessary training and conferences.
