PHYSICAL THERAPIST WJM Professionals is recruiting a Physical Therapist to provide personal medical services normally granted to a board-certified Physical Therapist. The Contractor shall perform a full range of professional Physical Therapy concepts and practices on-site for a variety of patients. The Contractor shall provide a full range of Physical Therapy services commensurate with the capabilities of the clinic including compiling a history and providing detailed recommendations/instructions to include, but not limited to:
REQUIREMENTS: - History, detailed examination, testing, and evaluation of the management of common musculoskeletal conditions. Provide manual muscle and joint testing, total musculoskeletal range of motion evaluations, and diagnosis.
- Treatment plan, therapeutic exercise, neuromuscular reeducation, functional activities, gait training, orthotics training, prosthetic training, kinetic activity training, electrical stimulation, iontophoresis, phonophoresis, manual traction, mechanical traction, massage, contrast baths, ultrasound, applying hot and cold packs, and vasopneumatic devices and whirlpool diathermy.
- Case consultation with referring Medical Officer.
- Post-surgical care consultations from referring medical officers.
- Provide immediate evaluation and management of emergent problems, as they occur, in addition to regularly scheduled appointments.
- Provide individual counseling about the effects and benefits of prescribed exercise regimens, physical fitness, and other related programs.
- Participate in the Health Services Quality Improvement Program.
- Provide instruction to clinic staff on an ad hoc basis.
QUALIFICATIONS: The Physical Therapist shall possess the following:
- A diploma from an accredited school of Physical Therapy.
- A current unrestricted active license to practice Physical Therapy from one of the 50 states, District of Columbia, Puerto Rico, Guam, or the U. S. Virgin Islands;
- A current certification in the field of Physical Therapy. Preferred orthopedics or sports board certification but not required.
- No unfavorable information on file rating with the National Practitioners Data Bank (NPDB).
- A current certification in Basic Cardiac Life Support (BLS) for the Health Care Provider by the American Heart Association (AHA), American Red Cross (ARC), American Safety and Health Institute (ASHI), or the American College of Emergency Physicians (ACEP).
- A minimum of two (2) years of recent experience (immediately preceding the submission of this offer).
- All applicants shall have at least 3 years' experience working in outpatient orthopedics.
CREDENTIAL REVIEW: Before commencement of this agreement, the Contractor shall submit the following credentials to be reviewed for technical acceptability. A current curriculum vitae including AT LEAST the following information:
- Degrees earned.
- Career History.
- Awards and certifications received.
- Professional Training/Courses attended in the past two (2) years.
- Professional organization affiliation and memberships; and
- Previous work reference(s) including name of the organization(s), number of years services rendered, address, and name of the point of contact with the telephone number.
- Date of birth (to be used only for independent query of the NPDB.
- Social security number.
- Evidence that active Physical Therapy has been practiced during 3 of the last 5 years immediately preceding performance under this contract.
- No adverse information on file with the National Practitioners Data Bank (NPDB).
REQUIRED COPIES: - A diploma from an accredited school of Physical Therapy.
- A current unrestricted active license to practice Physical Therapy from one of the 50 states, the District of Columbia, Puerto Rico, Guam, or the U. S. Virgin Islands.
- A list of any malpractice claims filed, pending or settled, and/or any other adverse professional actions, that involve the contractor.
- A current certification from the American Physical Therapy Association.
- A current certification in Basic Cardiac Life Support (BLS) for the Health Care Provider by the American Heart Association (AHA), American Red Cross (ARC), American Safety and Health Institute (ASHI), or the American College of Emergency Physicians (ACEP).
- Three letters of recommendation from previous employers.
- Evidence of immunity to Hepatitis A and B and evidence of freedom from chronic communicable disease(s) i.e., HIV, TB, and Sexual transmitted diseases (within 30 days of start of work);
