Nurse Family Partnership (NFP) nurses are responsible for the health of first-time mothers and their babies in Linn County through intensive home visiting services. Applicant will need to perform a variety of community health nursing duties, participating actively in program planning, representing community health nursing in community groups by providing nursing expertise and support. Also, must have the ability to provide highly skilled professional nursing in a designated area or specialty such as community health and home visiting and have the ability to utilize the nursing process in the establishment of care for our clients. Assess, plan, evaluate and implement general community health nursing service in assigned area of responsibility or special program.Make home, office or telehealth visits, giving direct patient care within the scope of standing orders, explaining basic concepts of prevention of disease and promotion of mental and physical health; teaching and demonstrating to patients and families nursing care, and assisting families in understanding health needs and methods of meeting needs.Plan and exchange information with physicians and other health workers for more effective care of the individual patient; take responsibility for development of community organization and planning for health services with community groups.Maintain records for nursing services to families and clinics, and make reports as required. For Nurse Family Partnership the RN will perform these additional job responsibilities:Apply theories and principles integral to implementation of the evidence based NFP program, including meeting the client in the home or a community settingMeet training requirements and practices with fidelity to the NFP model elements.Uses the nursing process to deliver individualized client care and set goals across six domains; personal health, environmental health, life course development, maternal health, family and friends, health and human services.Establishes therapeutic relationships with clients.Participates in on-going quality improvement and reports from data systems to guide and improve practice.Uses reflective supervision to evaluate and improve practice and takes part in self-care activities. Minimum Qualifications Completed Training for the Nurse Family Partnership program and at least 2 years' experience as a Nurse Home Visitor.Qualified candidate must possess a license to practice as a Registered Nurse in the State of Oregon. Possession of a valid motor vehicle operator's license and an acceptable driving record at the time of appointment is required.
