The National Nurse-Led Care Consortium (NNCC) is a national 501(c)(3) nonprofit dedicated to advancing nurse-led models of care through policy, technical assistance and consultation, and innovative programming. The Public Health Project Coordinator is responsible for one or more public health projects for NNCC. These projects may be funded by various sources, including federal and state grants and private
corporations. The Coordinator is responsible for the day-to-day activities of the programs and supports reporting responsibilities. This position will coordinate project planning and execution; work with stakeholders, clients, and home visiting staff; and provide data entry assistance towards tracking deliverables and monitoring performance measures. This position will primarily support the Family Advocacy
and Integrated Resources (FAIR) project. The portfolio of projects may be subject to change as projects evolve and/or new funding opportunities arise. This position is located in Philadelphia, PA. This position reports to the FAIR Project Manager.
RESPONSIBILITIES:
Coordinate administrative, programmatic, and grant management activities of public health projects. Act as a liaison with funding agencies, project partners, and other critical stakeholders. Work with supervisor to meet all reporting requirements. Adhere to program policies and procedures. Work with supervisor to create and manage service provider contracts to meet contractual deliverables. Provide support for invoicing and financial accounting processes in coordination with the Director of Nurse Advocacy and Resources. Work
collaboratively with program staff. Supervise interns as needed. Provide support for grant applications and side projects as needed. Ensure program quality.
FAIR Project
The Coordinator will be working on a new initiative of NNCC to connect families served by home visiting programs across Philadelphia to civil legal services and funds for concrete needs. The Coordinator work closely with the FAIR Project Manager and the Director of Nurse Advocacy and Resources, as well as the Fund Manager at Public Health Management Corporation (PHMC), to conduct complex case navigation for clients and to ensure NNCC provides timely assistance to families experiencing urgent needs. The
Coordinator will be an essential member of the FAIR project and will help to scale access to Family Support Funds and legal services for families with young children, while ensuring proper stewardship of resources.
Scope of Work:
? Support FAIR Project Manager and Director of Nurse Advocacy and Resources with execution of project deliverables including ongoing updates, meeting logistics, complex case navigation, and stakeholder relationships.
? Support the Project Manager and Director of Nurse Advocacy and Resources in content development and delivery of training activities related to the FAIR project.
? Prepare agenda for recurring NNCC project team meetings. Draft and disseminate agenda, take meeting notes, and share notes with team post meeting.
? Coordinate with members Public Health Management Corporation (PHMC)'s finance teams to regularly review and guide updates to program operations and best practices.
? Facilitate contracts management and invoice submission and tracking via PHMC's Document Management System (DMS).
? Support additional community education events as needed, including development and dissemination of educational materials.
? Meet with NNCC project team weekly for program status updates.
? Track milestones and due dates for project deliverables.
? Support preparation of regular reports for funder.
? Develop and implement trauma-informed FAIR project workflows with accuracy while proactively engaging in continuous quality improvement.
? Engage with internal and external stakeholders to efficiently operate FAIR according to funder requirements.
? Actively engage in specialized training related to home visiting, early childhood learning, social supports, public health, and legal aid and related topics critical to effective resource integration.

EXPERIENCE:

EDUCATION REQUIREMENT:
? Bachelor's Degree in related field required.
SALARY:
Pay Grade 18
