Minimum Education, Training & Experience & Special RequirementsBachelor's degree in public health, public administration, health education, health promotion or closely related health field AND one (1) year experience in public health or a related field; OR any equivalent combination of education and experience which provides the required knowledge, skills and abilities. Special requirements: Must pass background check. Valid driver's license and insurable under county liability coverage. May require use of personal vehicle or equivalent transportation for official business. - Supports with intervention identification, program development, and community engagement efforts to address community health needs.
- Coordinates the development and implementation of HealthyJoCo, the JCPH Community Health Needs Assessment Planning process and the Community Health Improvement Plan.
- Acts as JCPH liaison with community partners as assigned, which may include: land use plan development, alternative transportation, parks and recreation, and similar city or county plans.
- Assists in developing and implementing engagement processes that assure the informational exchange necessary to achieve appropriate plans and policies as well as identify potential barriers to new policy development and implementation.
- Increases the Performance Management and Quality Improvement (PMQI) capacity in order to ensure that public health goals are met.
- Increases JCPH's capacity to evaluate and improve the effectiveness of their organizations, practices, partnerships, programs, use of resources, and the impact the systems' improvements had on the public's health.
- Implements a performance management and improvement process that leads to a positive and measurable impact on the public health system.
- Coordinates JCPH's performance and quality initiatives to ensure alignment and optimal use of resources.
- Identifies best performance and quality improvement practices, and makes recommendations for implementation.
- Assists with development of university undergraduate and graduate students completing internships and practicum experiences at JCPH.
- Represents the Department and participates in training/education opportunities, conferences, seminars and work-related meetings as requested.
- Performs all work activities in accordance with county policies, procedures, and safety practices.
