LOCATION: Syosset, NY REPORTS TO: QI Director - Family and Residential Foster Care DEPARTMENT: PQI SCHEDULE: Full Time / Hybrid AGENCY BACKGROUND: MercyFirst is a not-for-profit human and social service agency that has been serving children and families in need since we were founded by the Sisters of Mercy/Hermanas de las Misericordia in 1894. Today our agency continues to address the emotional and physical needs of children and families in Brooklyn, Queens and across Long Island through innovative treatments and life-changing interventions. We provide community-based prevention and family foster care services, group homes in the community for struggling children and families within the child welfare and juvenile justice systems, and short-term residential services for unaccompanied migrant children. Each year, MercyFirst serves more than 3,000 children, teenagers and families overcome enormous obstacles, re-imagine their futures and develop their full potential. PROGRAM BACKGROUND: The Performance and Quality Improvement Department is responsible for the agency's information, program and operational quality, compliance, program data tracking, reporting and quality/performance improvement processes across MercyFirst. PQI ensures MercyFirst meets all regulatory and contractual program performance and facility requirements across the agency. Led by the Chief Quality Officer, PQI staff support programs the above areas along with conducting continuous quality improvement activities such as case record reviews, utilization reviews, stakeholder surveys, KPIs and outcome measures. SUMMARY: MercyFirst is currently seeking a QI Coordinator of Residential Programs to be responsible for helping to ensure our children and families have a beneficial experience while in our care. We strive to maximize positive outcomes for our families. We monitor regulatory compliance and measure performance for best practices. This position occasionally travels to Queens and Brooklyn sites from Syosset to ensure our group homes are safe, pleasant environments and to get feedback from our clients on their experiences. REQUIRED QUALIFICATIONS:
  • Associates in health, public health, policy or related field required. A Bachelor's Degree in health, public health, policy or related field preferred.
  • Relevant training and/or experience in child protection, child welfare and/or any other residential treatment type setting. Knowledge of NYC child welfare programs, NYC ACS, NYS OCFS, NYS OMH, and other state/federal systems a plus. Demonstrated ability in analytic and critical thinking skills. Excellent oral and written communication skills. Computer proficiency in Word, Excel, Access, and PowerPoint with ability to learn NYS CONNECTIONS, NYS Data Warehouse and ASARA.
  • Understanding and willingness to commit to the agency's Mission, core values, Sanctuary Commitments and Diversity, Equity, Inclusivity, and Belonging (DEIB).
  • Must possess a valid driver's license.
RESPONSIBILITIES:
  • Tracks environment of care documents including Fire and Safety inspections for all agency sites.
  • Maintains monthly inspection reports, generates, and shares reports with relevant program and administrative staff.
  • Monitors fire drill compliance and ensures that drills are conducted according to guidelines and program schedule.
  • Tracks monthly Resident Food Advisory Meetings and ensures that meeting notes are taken and maintained.
  • Manages Residential daily movement entries in CONNECTIONS Activities (CCRS).
  • Monitor and present Residential casework quarterly compliance, including but not limited to FASPs, PYA and QRTP Aftercare.
  • Maintains Residential Casework Contact list and ensure accordance with ACS and OCFS requirements.
  • Tracks and maintains Residential requests for vital documents.
  • Tracks Critical Incidents and generates daily and monthly reports to share with relevant program and administrative staff
  • Complies with all federal, state, and agency security and privacy policies intended to protect client confidentiality and client health information. Oversees Residential Care survey and monitors both the administration, data collection, and reporting.
  • Cooperates with other Program Quality Improvement team members in cross-program projects such as case record reviews.
  • Any and all other duties issued by the QI Director.
