We are an equal opportunity employer.Empowering People's Independence (EPI) provides support for people with developmental disabilities, epilepsy, and brain injury through progressive, tailored care to improve their health, wellbeing, independence, and social inclusion.Job Purpose:The primary function of this role is to oversee the day-to-day implementation of the Staff Action Plans (SAP)/Plans of Care (POC) in the Community/Day Habilitation/Respite/Residential programs, established by providers, and to ensure quality and Medicaid compliance standards are met through verification of all completed Office for People with Developmental Disabilities (OPWDD) and Department of Health (DOH) required documents.Essential Functions:Ensures compliance with all OPWDD and DOH regulations.Creates Staff Action Plans/Service Plans in collaboration with persons supported/advocates & directs support professionals, trains staff on how to implement plans accordingly to achieve desired goals.Assures the timely and accurate completion of Staff Action Plans/Service Plans for persons receiving supports and that they are distributed per OPWDD/DOH regulation.Observes persons supported, reviews data and progress via daily notes, and revises the Staff Action Plans/Service Plans based on individual need and performance.Maintains dates for required Staff Action Plans/Service Plans, Life Plans/Plans of Care, Individualized Plan of Protection (IPOP), Plan of Nursing (PON), Level of Care Eligibility Determination (LCED), etc., and assures all record keeping and documentation requirements are completed timely to meet billing, quality, and Medicaid compliance standards.Acts as a member of the team for each person supported by participating in Life Plan/Plan of Care meetings, which must be attended in person or virtually, to evaluate and discuss services provided.Ensures all yearly paperwork is complete.Consults with Program Managers/Supervisors to ensure consistency of services.Serves as a positive role model for persons supported and other staff. Interacts with others in a way which will assist persons in developing behaviors, skills, and attitudes necessary for successful community living. Promotes positive respectful interactions with direct support professionals and persons supported.Compliance:Must maintain confidentiality and exercise discretion concerning all business-related matters, including but not limited to board/vendors/donors' names, investigations, finance, operations, and employee relations matters.Must complete specific company-required trainings as mandated by our governing agency, Office for People with Developmental Disabilities (OPWDD), Department of Health (DOH) within 90 days of hire and annually thereafter.Must always adhere to all departmental/company policies & procedures.Professional Deportment:Consistently demonstrate EPI's Values:IntegrityCourageRespectAdhere to all company/departmental policies & procedures and ensure departmental compliance.Non-Essential Functions:Performs other duties as assigned by Senior Compliance Coordinator, Community Services Management team/Designee, within scope of practice.Participate on various committees as time permits.Education and Experience:Bachelor's Degree in a related field and one year experience in the human service field with Qualified Intellectual Disabilities Professional (QIDP) status. PLUS at least one year working with people diagnosed as developmentally disabled.Must have at least one year of experience working directly with people with intellectual/developmental disabilities.Qualifications:Must have at least one year of experience working directly with people with intellectual/developmental disabilities.A Bachelor's degree in a Human Service related field is REQUIRED.Must possess and maintain a valid NYS Driver's License - a vehicle is required for business travel.Mature interpersonal/professional skills to effectively collaborate & resolve issues.Demonstrate poise, tact, and diplomacy during difficult and/or stressful situations.Must be able to read, write and speak English.Proven ability to work in a demanding environment, with demonstrated ability to manage multiple tasks, and customer requests, while meeting deadlines.Strong working knowledge of Microsoft programs to include Email, Word, Excel, PowerPoint, Teams, and electronic medical record systems (e.g., MediSked).Physical Demands:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
