Your skill set provides for health, safety, accountability, supervision, and training for people supported in the community. Qualifications:
- Bachelor's degree in human services or in a related field
- Valid driver's license with acceptable driving record
- 2 years' experience with persons with Intellectual Developmental Disabilities
Requirements:Your Role: You will be responsible for overseeing the essential areas required to effectively support the program plans for individuals served in Greenville and Wilmington, NC. The Qualified Professional (QP) will facilitate and promote the Interdisciplinary Team Process, which includes the development of CLC's service objectives and the individualized Service Plans (ISP) for clients. Collaboration with other team members and Managed Care Organization (MCO) CARE Coordinators is vital to ensure the efficient delivery of services to those supported. You will also be required to adhere to all documentation requirements for the Interdisciplinary Service Plans (IPS), ensuring that all services are properly recorded in accordance with the standards set by the North Carolina Department of Health and Human Services, the Managed Care Organization, and any additional assigned responsibilities.Our purpose is to champion the power of human connection, and the heart of connection is inclusion.
