Position Description

Position Title: Quality Assurance/ Infection Control Nurse

Summary: The Quality Assurance/Infection Control Nurse (LPN/RN) is responsible for taking a proactive approach to maintain the quality program. Performs chart audits, assists with chronic care management, and creates quality reports to ensure compliance with federal and state laws and regulations. Leads the Infection Control Committee and works collaboratively with providers and clinical staff to decrease the potential and actual risk of infection.

Classification: Non- Exempt, Full-Time/ Salary

Supervision: Reports to the Director of Quality

Completion of all compliance training and educational programs is a requirement of employment.

Responsibilities:

* Maintains strict confidentiality at all times

* Performs patient observations utilizing the nursing process

* Documents all care provided according to policy

* Performs treatments, procedures as prescribed by the Provider

* Responds to patient/family/caregiver needs

* Responds to crisis situations according to policy

* Utilizes supplies in a cost-effective manner

* Performs clinic and outreach related tasks

* Organize the Infection Control Committee and maintain all meeting minutes

* Ensure the infection prevention and control procedures meet CMS, county and state,

APIC, and CDC regulations, standards or guidelines

* Provide consultation, education, and training to staff related to quality and infection

control

* Responsible for the Infection Control Program including surveillance and analysis of data

* Evaluate the effectiveness of the Infection Prevention and Control Plan annually

* Conduct risk assessments at least annually or as needed

* Develop and implement improved patient care procedures related to quality and infection

Prevention and control

* Participates in outcomes from the Performance Improvement Program

* Establishes and maintains professional working relationship with other members of the health care team

* Participates in continuing education and professional growth

* Attends staff meetings

* Responsible for policies and procedures related to infection control

* Maintains knowledge of, and complies with all organizational policies and procedures

* Assists other team members as needed

* Adhere to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG Guidelines, other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, and Privacy Officer any suspected unethical, fraudulent, or unlawful acts or practices

* Performs other duties as assigned

Essential Functions:

Physical Factors

Lifting: up to 30 pounds occasionally

Carrying: up to 30 pounds occasionally

Standing: 30-60 minutes at a time throughout the day

Walking: 5-10 minutes at a time throughout the day

Sitting: 15-120 minutes at a time throughout the day

Driving: 10 minutes to 4 hours at a time routinely

Vision: color distinction necessary

Hearing: no greater loss than 40dB average for 500, 1000, 2000 Hz

Qualifications:

* Active Ohio Registered Nurse License or License Practical Nurse License

* Bachelors degree (BSN) highly desirable but not required

* Minimum of 3 years experience in healthcare preferred

* Current Basic Life Support (BLS) certification

* Must be proficient and have a strong working knowledge of Microsoft Office (365, Excel, Word, etc.)

* Excellent oral and written communication skills

* Must be able to multi-task

* Must be well organized and detail oriented

* Ability to work with a culturally diverse group of people

* Must have current drivers license, and provide proof of valid auto insurance
