SUMMARY:
The Quality Management Coordinator is responsible for the coordination of quality management at various campuses. The primary function of this position is to maintain concurrent review and intervention in the timely completion of recognized best practice indicators as identified by regulatory agencies and hospital quality improvement priorities. The coordinator also prepares and reports Southwell Medical Campus quality performance.
RESPONSIBILITIES:
* Applies the appropriate QI tools and methods for successful quality and process improvement projects.
* Attends meetings as required and participates on committees and teams as directed.
* Completes the collection and entry into data base systems of all data required to risk stratify patients according to NNIS standards prior to patient discharge.
* Designs data collection plans for key performance indicators.
* Develops and maintains professional working relationships with physicians, nursing staff, and other health care providers.
* Develops and reports discrepancies and outcomes.
* Participates in continuous survey readiness activities.
* Facilitates the documentation of identified indicators and care delivery consistent with evidence based practices and regulatory requirements.
* Identifies and reports patient safety risks and events.
* Initiates case finding according to pre-determined criteria to identify patients that are appropriate for quality indicator monitoring.
* Integrates data from internal and external electronic systems.
* Integrates ethical standards into practice.
* Intervenes concurrently as able to ensure compliance with care provision in accordance with evidence based practices and quality indicator measures.
* Maintains information and current knowledge of quality indicator measures, resource management, performance improvement, patient education, and discharge planning processes, including philosophy and principles.

* Operationalizes processes to support compliance with standards.
* Protects privileged or confidential information.
* Serves as an active member of the Safety Committees and attends other committees as deemed appropriate to perform job duties.
* Serves as integral part of care improvement team by coordinating quality improvement initiatives utilizing proven quality improvement principles, bringing suggestions for improvement in processes to ensure compliance with quality indicators, and management and feedback of team performance data.
* Supports the quality infrastructure.
* Keeps abreast of pertinent federal, and state regulations and laws and Tift Regional Health System, Inc. ( TRHS ) policies as they presently exist and as they change or are modified.
* Understands and adheres to: TRHS' compliance standards as they appear in TRHS's Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy; and HIPAA and TRHS policies regarding privacy and security of protected health information.
* Demonstrates the ability to perform tasks that meet the age-specific requirements of the persons, patients, vendors, and staff that the employee is charged to interact with as required by the position.
* Offers suggestions on ways to improve operations of department and reduce costs.
* Attends all mandatory education programs.
* Improves self-knowledge through voluntarily attending continuing education/certification classes.
* Maintains required competency levels as identified in written exams, skills checklists, skills labs, annual safety and health requirements as well as service excellence education hours requirements.
* Cross-trains in order to better assist co-workers and to provide maximum efficiency in the department.
* Volunteers/participates on hospital committees, functions, and department projects.
* Manages resources effectively.
* Reports equipment in need of repair in order to extend life of equipment and removes malfunctioning equipment out of service with timely reporting to the appropriate personnel.
* Makes good use of time so as to not create needless overtime.
EDUCATION:
* Bachelor's Degree in Nursing
CREDENTIALS:
* Registered Nurse
OTHER INFORMATION:
* Bachelor's degree in nursing plus five (5) years' experience in healthcare required.
* Experience with computer programs, data management, and documentation to regulatory requirements preferred.
* Certified Professional in Healthcare Quality credential preferred.
* Skilled Nursing and/or Rehabilitation experience, including knowledge of the CMS Minimum Data Set Resident Assessment preferred.
