The Quality Coordinator will be responsible for completing in-home visits to help meet patients' needs. Visit focuses involve point-of-care testing including diabetic A1c collection, blood pressure checks, colorectal screening kits, and other testing required by HEDIS/Stars measures. This role is essential to our organization's goal of improving the overall experience of patient care, including the quality of health, and work to reduce the cost of care. For this field-based position, individuals must be able to travel up to 80% of the time in their own personal vehicle. This person will be traveling to patients' homes to ensure their quality of care.Essential Duties and Responsibilities:
- This position is responsible for performing in-home visits with patients
- Review data collected to confirm gaps in care are captured
- Build positive, professional relationships with the patients we serve
- Explain point-of-care testing and reasoning to patients
- Demonstrate excellent communication and collaboration skills
- This is both an in-office/field position: candidate must be comfortable traveling up to 80% of the time
Qualifications and Skills:
- High school diploma or GED equivalent required
- Must have their own reliable vehicle to drive out in the field to patients homes
- Medical Assistant (CMA) Certification and or Certified Nurse Assistant (CNA) Certification
- Current CPR required
- 2+ years' experience in a healthcare environment
- Ability to perform point-of-care testing such as urine sample collection, A1c checks, blood pressure checks
- Familiar with medical terminology
