The Quality Coordinator will be responsible for coordinating, scheduling, and following up on clinical care needs for patients as it relates to Healthcare Effectiveness Data and Information Set (HEDIS) and other quality STAR initiatives. This performance driven role has a primary function to conduct in-home Point of Care testing for care gap closure, this will also include entering data into our data base to capture/reconcile all Point of Care (POC) tests being performed. The position contributes to the organization's drive to improve the overall experience of patient care, including the quality of health, and work to reduce the cost of care across the population served. The Quality Coordinator LPN also works in close collaboration with the RN Care Management team to fulfill the care aspects of the quality department.Essential Duties and Responsibilities:
  • Retrieving and reviewing data collected to confirm gaps in care are captured
  • Combines data from all systems for accurate precise reporting
  • Performs general nursing assessment and utilizes the results to determine additional patient needs in collaboration with the RN Case Management team
  • Utilizing nursing knowledge, skills, and abilities to care for people in a variety of settings. This may include maintenance of health and prevention of illness and assessing patient responses to actual or potential problems
  • Educating patients on procedures that are being scheduled/performed and the reason for performing them
  • Participates in orientation and training of new staff
  • Pursue company and departmental procedures, practices, policies, and guidelines
  • Assist with scheduling, and outreach, including all measures if indicated by the Quality Director
  • Performs member call backs for abnormal results
  • Processes/schedules referral appointment with Specialist
  • Process and fax all medical records requests and POC forms to PCP offices when approved.
  • Participation in quarterly overtime and some weekends when approved
  • Conduct data entry and maintain appropriate documentation and tracking for quality initiatives
  • Coordinate and order preventative and diagnostic tests around gaps in care on behalf of the primary care physician utilizing standing orders/protocols required by the health plan
  • Conducts in-home member visits for POC testing, with specimen delivery to the lab when applicable
  • Coordinates any pharmacy medication pickup and delivery to members as needed
  • Assist in improving operation workflows and streamlining processes
  • Care Manage abnormal results to promote compliance and follow through under the direction of the RNCMs
  • Order and track needed supplies for POC testing
  • Assist with field training for new hires
  • Assist with field medication reconciliation
  • Help navigate patients needing assistance with medication adherence and regimens
Qualifications and Skills:
  • High school diploma or GED equivalent required
  • 2+ years' experience in a healthcare environment
  • Active CNA license in applicable states
  • Recent and related experience working with HEDIS/STARS program
  • Experience and knowledge with CPT II codes and diagnosis
  • Demonstrate the ability to work collaboratively, efficiently, and effectively as a high performing quality team member.
  • Strong ability to self-manage time to manage multiple projects and priorities in a fast-paced environment
  • Excellent organizational and interpersonal skills
  • Ability to handle multiple tasks, set priorities and develop action plans
  • Experience in Electronic Health Records
  • Ability to collect and process specimen samples
  • Experience with verifying Eligibility and navigating through Insurance Portals
  • Basic knowledge HCC's and Medical Diagnosis
  • Ability to perform multiple procedures (i.e.) Diabetic Retinal Exams, Urine Dips, FIT Kits
  • Familiarity of Medicare guidelines to perform patient screening's
  • Medical Terminology knowledge
