Rochester Rehab & Living is seeking a Quality Coordinator RN to join our caring team! The QAA/QAPI Coordinator is responsible for facilitating the implementation and maintenance of Performance Improvement Incentive Projects (MOVE and STREAM). The process provides on-going performance improvement and improved outcomes Salary Range: $38 - $42 (Based on experience) Pay in Lieu Available Schedule: This is a PT Position - Schedule TBD (Occasional Weekend Availability) Quality Coordinator Nurse Highlights: * 403(b) Retirement Plan * Career scholarships available * Quality training, continuing career education and leadership programs * Medical, Dental and Vision Insurance * Paid Time Off (Vacation, Holiday & Sick Days) About the Job: The QAA/QAPI Coordinator will utilize comprehensive clinical assessment/observation skills to understand causation of resident conditions, to support resident mind health and mobility, and assist in the creation of resident centered care plans that will impact meaningful change. Essentials: MOVE PIPP: * Utilize clinical assessment and critical thinking skills for MOVE FORWARD Program tools. * Provide program education to staff, resident and family members. * Lead the MOVE FORWARD Teams and provide direction, support and resources as required. * Conduct facility resident and staff Resiliency Groups. * Conduct and coordinate the Staff Bias Awareness Tool and assess and analyze findings with the MOVE FORWARD PIPP team and implement appropriate interventions with leadership. * Interact with multiple stakeholders to deploy program goals and assess, align, and implement interventions. * Interpret and analyze facility QIs and QOL measures and assess and develop interventions for on-going improvement and sustainment. * Partner with the Empira Liaison and SMEs to successfully deploy program in the facility. * Develop and nurture a trusting relationship with all stakeholders. * Report to the multiple stakeholders on program goals and outcomes including, but not limited to: QAPI, IDT, Medical Provider Group, Resident and Family Council. * Effectively use program tools including assessments, audits, educational resources and technology. * Ability to work flexible hours to accommodate all stakeholders. * Ability to travel 10% of the time for program needs. * Attend required meetings and trainings. STREAM Sustainment Plan oversight: * Conduct clinical assessments/observations using root cause analysis to understand causation of resident conditions. * Utilization of technological resources to gather objective data to inform assessments/observations (e.g. bladder scanner, Identifi, Actigraphy, etc.). * Utilize interviews of staff, resident (or responsible party) to gather subjective data to inform assessments/observations and uphold resident preferences. * Discover and address knowledge gaps of resident, family and staff members. * Review medication alignment as part of routine assessments/observations. * Design care routines that support restorative sleep at night and active engagement during the day. * Create and implement a care plan jointly with the care team, provider, resident, and their loved ones based on what matters most to the resident. * Communicate care plans to the care team. * Evaluate and monitor the care plans. * * Quality Improvement: * Assist and support clinical leadership with quality improvement initiatives as directed (ex: QIIP, PIPP, QAPI). * Work in coordination with MDS nurse to support the facility performance measures/ outcomes. * Nursing Documentation Support: * Audit documentation for completeness and accuracy. * Mentor and train staff on timely and appropriate documentation in the EMR. * Coach and support staff on restorative care plan approaches. Required Qualifications: * Registered Nurse OR Licensed Practical Nurse (under direction of an RN) * Numerical ability necessary to work with charts and graphs and other tools of QAA/QAPI Coordinator.
