ORGANIZATION: Citizen Care Inc. DEPARTMENT: Administration POSITION: Quality Improvement Coordinator STATUS: Full-Time SCHEDULE: Flexible to meet the needs of the program (primarily daylight) LOCATION: Robinson Township SALARY: $58,000 - $62,000 per year

Citizen Care Inc., a subsidiary of Partners For Quality Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit organization, accredited by the Council on Quality & Leadership. Citizen Care has locations throughout the West Hills and South Hills of Allegheny County.

Responsibilities:

Responsible to the Executive Director of Citizen Care for the overall management of quality assurance activities for Citizen Care, a subsidiary of Partners For Quality, Inc. All responsibilities shall be completed in a manner consistent with the Vision, Mission and Values of Partners For Quality, Inc.

Incident Management

  • Coordination and direction of the Citizen Care Incident Management process. This includes daily monitoring of the Home and Community Services Information System (HCSIS) for new incidents.
  • Obtains Certified Investigator training through the Office of Developmental Programs (ODP) and conducts assigned Certified Investigations of a very high risk nature i.e., deaths or other highly sensitive situations.
  • Implements new ODP protocols and regulations related to risk management.
  • Communicates, both verbally and in written reports, across Community Living divisions, Community and Employment Services, Life Sharing and Partners For Quality (PFQ) corporate to ensure that all stakeholders are aware of very serious incidents, trends reporting, and corrective action plan completion.
  • Implements systemic plan of action for proactive quality improvements to program outcomes.
  • Monitors corrective action in collaboration with PFQ Human Resources (HR) and PFQ Compliance and all CCI
  • Co-Chairperson of Basic Assurance Monitoring (BAM) peer review committee.
  • Reviews preventative and corrective action and monitors for implementation and completion. Reports all information to PFQ Compliance.
  • Monitors and maintains Enkompas dashboards for all incidents within CCI. External reports pulled monthly from the Enterprise Incident Management (EIM) to be uploaded into databases. Might be a PFQ Function
  • Monitors Electronic Health Record (EHR) in conjunction with team members for accuracy and completion.
  • Accreditation/Licensing

  • Assists the Executive Director in maintaining various Council on Quality and Leadership (CQL) databases, and Citizen Cares compliance with existing accreditation standards.
  • Participates on BAM Committee. Completes Personal Outcomes Measures Interviews and assists with scheduling interviews, as needed.
  • Upkeeps CQL goals and plans in line with 4 year accreditation.
  • Assists in Citizen Cares compliance with ODPs newly implemented Quality Assurance & Improvement (QA&I) process. Maintains portions of agencys Quality Management Plan in web-based format.
  • Obtains and upkeeps quality management training through ODP.
  • Assists with Coordination of State Licensing features, and helps maintain service standards to meet the QA&I requirements. Builds data reports to show compliance with mandates.
  • Serves as EIM primary point of contact and incident management point person.
  • Responds to any incidents reviewed and needing updated through the fidelity dashboard review implemented by ODP.
  • Other duties as assigned.

    Qualifications:
  • Bachelors degree in Human Services and six (6) years of direct and managerial experience working with individuals with disabilities
  • OR

    Masters degree in Human Services and three (3) years of progressive experience working with individuals with disabilities

  • Minimum of three (3) years experience in the human services field
  • Minimum of two (2) years management experience
  • Knowledge and understanding of regulatory guidelines for programs, incident management protocols and quality assurance, focusing most heavily on the IDD system.
  • Must have current ODP Certified Investigator certification/training
  • Excellent communication, both verbal and written
  • Excellent computer skills
  • Excellent references, both personal and professional
  • A valid Pennsylvania, Ohio, or West Virginia drivers license
  • Eligible for Act 33, 34 and FBI clearances
  • Minimum Compensation: USD $58,000.00/Yr. Maximum Compensation: USD $62,000.00/Yr.
