As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y.Jordan Health is now seeking a Quality Improvement Director who, under the guidance of the Chief Medical Officer, is primarily responsible for providing population health expertise and leadership in the collection and analysis of data to collaborate with a variety of staff to optimize clinical quality, patient safety, and fiscal performance. Jordan Health's fiscal performance is improved. Clinicians and staff gain the insight they need to reduce costs, increase revenue and improve patient safety and outcomes while complying with regulations and standards. The QI department staff are fully supported and equipped to meet/exceed performance standards. Education Required: Bachelor's level Health-related degree preferred, such as RN, at minimum preferred. Graduate-level degree in health administration, public health, epidemiology or equivalent preferred. Experience Required: 2-5 years of successful experience in a health population, quality improvement setting or related field. An appropriate combination of education, skills, and experience, can be substituted for total years; Proven leadership and management skills. Healthcare background, experience and performance that promotes a high level of credibility with clinical professionals. Ability to use analytical tools and techniques to investigate data trends, issues, and to draw conclusion from data. Knowledge and application of industry standard report writing tools, such as Excel, SQL, Crystal Reports, etc. Excellent verbal and written communication skills and a capacity for translating business needs into clinical specifications, and vice versa. High level of work knowledge and understanding of eClinicalWorks or other electronic health record (EHR). Experience with ICD and CPT coding. Special Skills, Knowledge Required: Excellent written, oral, and interpersonal skills. Highly self-motivated, directed and change oriented. Good analytical, problem-solving skills and ability to effectively prioritize and execute tasks under pressure. Able to work, contribute to and flourish in a team centric, collaborative environment. Able to represent Jordan Health, when requested, and keep senior staff informed and engaged in the work of the community at large, our IPA and practice association. Exhibits high ethical standards in all aspects of work.
