Job Title: Quality Management Coordinator - HIV/Health Services JOB DESCRIPTION:The position of Quality Management Coordinator - HIV/Health Services maintains a documented, ongoing Quality Management (QM) program. QM is comprised of systematic processes with identified leadership, accountability, and dedicated resources using data and measurable outcomes to determine progress toward relevant, evidence-based benchmark. QM Programs should focus on linkages, efficiencies, and provider and client expectations in addressing outcome improvement and should adapt to change. The QM program must include a QM committee and must meet a minimum of four (4) times annually, on a quarterly basis, and meet DSHS requirements for the QM Committee membership to have diverse representation. This position reports directly to the Program Manager of HIV/Health Services and through channels to the BVCOG Executive Director.DUTIES AND RESPONSIBILITIES: 1. Create, execute, and evaluate BVCOGs Quality Management (QM) Plan that outlines the activities and performance measures of the department. 2. Develop and disseminate policies and/or procedures necessary to support the execution of BVCOGs QM Plan. 3. Review and approve Subcontractor QM Plans and any supporting policies and procedures. 4. Monitor and evaluate compliance with Subcontractor QM Plans and supporting policies and/or procedures. 5. Represent BVCOG and its Subcontractors in the State-wide QM processes. 6. Facilitate BVCOGs Regional QM Committee process, including: - Providing performance measure data to the regional QM committee during their quarterly meetings - Ensuring the Regional QM committee identifies at least one Quality Improvement (QI) project based on health outcomes data feedback provided, and - Initiating at least one PDSA cycle of QI activities, and reports results to BVCOG. 7. Ensure that each Subcontractor identifies at least one QI activity based on health outcomes data feedback provided, initiates at least one PDSA cycle of QI activities and reports results to BVCOG. 8. Ensure that strategies for improvement to quality general and HIV-specific medical care include vital health-related supportive services in achieving appropriate access and adherence with HIV medical care and ensure that available demographic, clinical and health care utilization information is available and used to monitor the spectrum of HIV related illness and trends in the local epidemic. 9. Perform ongoing assessment of each Subcontractors QM program and whether it meets the expectations of funding sources. Provide technical assistance to any Subcontractor identified as not meeting expectations to revise and maintain a QM program that meets expectations of funding sources. 10. Provide annual technical assistance to Subcontractors in accordance with an established TA schedule and Subcontractor - specific TA, as needed/requested. 11. Other duties as assigned by the Program Manager.QUALIFICATIONS: Minimum of a bachelors degree with at least two years of previous experience and training in health/social service delivery. Prefer advanced degree and experience in public health/social service delivery/quality management.
