Job Description:Job Title: RCF Care CoordinatorLocation: Kirksville, MissouriDepartment: Residential ServicesEmployment Type: Full-TimeJob Overview: We are seeking a dedicated and resourceful RCF Care Coordinator to join our team in Kirksville, Missouri! In this role, you will assist clients in accessing necessary resources in the community, facilitate client admissions, manage census reporting, and work closely with the billing department. The position involves providing orientation regarding program expectations and offering community support services for individuals with mental health and/or substance use disorders. If you have a passion for helping individuals achieve the best outcomes and possess the necessary qualifications, we encourage you to apply!Position Perks & Benefits:* Competitive pay* Flexible schedule* Comprehensive benefits package* Positive and inclusive work cultureKey Responsibilities:* Perform the duties of a Community Support Specialist, including documenting services and interventions related to client treatment plans* Meet with existing clients to encourage utilization of offered services and provide case management as necessary* Develop and maintain community partnerships to help clients obtain medical care, insurance, transportation, and other essential services* Assist with the orientation of clients on their rights and program rules* Develop discharge and aftercare plans to ensure successful transitions to continued services* Ensure effective communication and coordination with other service providers, medical professionals, and referral sources* Complete all mandatory training within regulatory standards, maintaining a waiting list and managing program census as applicable* Respond to new referrals and maintain positive relationships with clients, coworkers, and referral sources* Facilitate screenings and coordinate admissions in electronic medical records and applicable databases* Provide exceptional customer service to create a welcoming environment for new clients/patients* Assist in filing, quantitative chart review, and compiling chart information as needed* Represent the agency professionally and adhere to the code of ethics and agency policiesRequirements:* Bachelor's degree in a human services field or a combination of higher education and qualifying experience in mental health or substance use services* Valid driver's license with an acceptable driving record and current auto insurance* Successful completion of a background check including criminal record, driving record, abuse/neglect, and fingerprint checks* Completion of New Hire Orientation and all required training within set timelines* Ability to communicate effectively both verbally and in writing, with strong organizational and time management skills* Knowledge of confidentiality laws and case management principles related to mental health and substance use disorders* Must be at least 21 years of age to operate company vehicles and at least 23 years of age to operate a 15-passenger vanBrightli is on a Mission: A mission to improve client care, reduce the financial burden of community mental health centers by sharing resources, a mission to have a larger voice in advocacy to increase access to mental health and substance use care in our communities, and a mission to evolve the behavioral health industry to better meet the needs of our clients.As a behavioral and community mental health provider, we value diversity, equity, and inclusion in our workforce and encourage applications from individuals from diverse backgrounds and experiences.
