Responsibilities:The primary responsibility of the Community Linkage Coordinator is to obtain all necessary benefits and prequalification for persons currently housed in a correctional facility, who as identified by their medical condition, require transition to a nursing home, assisted living, long-term care, or hospice environment. The CLC will ensure, once cleared by the correctional facility, prompt placement of the individual in an environment equipped to provide appropriate care for their medical needs in accordance with the rules and regulations of the facility.

Corrections
Below is a list of your responsibilities as a Community Linkage Coordinatorwith YesCare:
Reviews high-risk patient lists and determines if special needs replacement is required.
Responsible for establishing and maintaining a network of community resources for special needs patients (LTC, SNF, ALF, Rehab).
Assist in obtaining necessary information related to benefits and prequalification documentation to include payor authorization approval from insurance, Medicaid, or Medicare as required.
Collaborates with community resources as well as site physicians to procure appropriate services and placement for patients.
Coordinates with the courts, attorneys, and correctional administration to ensure prompt transition to the appropriate facility based on the patients medical needs.
Assists in planning and implementing the goals and objectives of programs and projects. May direct special projects as requested.
Provides comprehensive social work to restore the individual to their maximum functioning capacity.
Provides the necessary preparation of documentation, necessary records and reports.
Participates in and conducts in-service programs, as well as assists in the orientation of new staff.
Attends staff meetings.
Responsible for obtaining information for comprehensive psychosocial history.
Performs other related duties at the request of the Program Director.
