Job Title: Recovery Center Case Manager
Location: Phoenix, AZ (4 locations in the following zip codes: 85008, 85029, 85012, 85042)
Job Type: Direct Hire, Full-Time
Hours: Monday – Friday, 8:00 AM – 5:00 PM
Target Start Date: ASAP
Pay Rate: $16-24/hr based on behavioral health technician level
Sign-On Bonus: $2000 sign-on bonus paid out in two installments over 6 months
Job Summary: A leading healthcare organization is seeking a dedicated and compassionate Recovery Center Case Manager to join their interdisciplinary clinical team. This role involves working with individuals diagnosed with Serious Mental Illness or General Mental Health conditions under Court Ordered Treatment. The ideal candidate will assist clients in identifying and achieving their treatment and recovery goals, providing supportive services, and ensuring a safe recovery environment.
Key Responsibilities:
- Collaborate with clinical teams to engage, educate, and coordinate care with patients, their families, and various healthcare providers.
- Conduct face-to-face interactions with patients and their support systems in various settings, including homes, communities, and recovery clinics.
- Ensure ongoing communication and collaboration with all involved parties, including behavioral health, medical, dental providers, and community resources.
- Provide crisis intervention and support, promoting a recovery-focused environment.
- Participate in clinical staff meetings, case conferences, and other relevant meetings.
- Assist patients with transportation needs, as medically necessary.
- Deliver excellent customer service to patients, internal and external stakeholders.
- Develop and implement transition, discharge, and aftercare plans.
- Complete all case management functions, including assessments and service planning.
- Ensure appropriate discharge plans for hospitalized or incarcerated patients.
- Assist patients in connecting with physical health services and follow-up care.
- Maintain accurate and timely documentation in the Electronic Health Record.
Requirements:
- Minimum of a high school diploma or GED with 1.5 years of behavioral health experience, or an associate's degree in a related field, a bachelor's degree with relevant experience is preferred.
- Strong interpersonal and communication skills.
- Basic understanding of DSM-V preferred.
- Proficiency in Microsoft Office and electronic medical records.
- Valid driver's license and ability to meet driving policy requirements.
- Reliable transportation (ability to transport patients).
- in the office 50% of the time and traveling in the community 50% of the time.
- Mileage reimbursement is provided
- Must have previous experience working with serious mental illness (SMI)/behavioral health population.
Ideal Candidate:
Associate's degree minimum, higher education strongly preferred.Minimum 2 years' experience as BHT or Case Manager within behavioral health.Minimum 4+ years driving experience.- Nice to Haves: Nextgen experience.

- Clear 39-month driving record
- Must have or be able to obtain a Level 1 Fingerprint Clearance Card. Assistance will be provided to obtain the card, including covering the payment.
