This Full-Time role includes completing assessments, treatments, goals, and monitors patient's progress in a timely manner. In this position, the employee must have the ability to interpret evaluations from various disciplines (OT, PT, Speech) and use assessment skills to develop appropriate plan of care. Employee must have the ability to plan individual sessions for TBI patients as well as co-treating with other therapy team members as well as completing assessments of sub-acute rehab patients for leisure needs and provide programming and supplies. The focus for this position is to plan, organize and lead group activities and individual interventions designed to meet the recreation needs and interest of the residents. This position promotes the highest level of physical, mental and psychosocial functioning of the residents served.
- On a monthly basis, plan creative and resident-focused recreational activities that support self-esteem and dignity (e.g. age appropriate), are tailored to the unique preferences and abilities of residents, are stimulating, and challenge residents to realize their full potential.
- Complete Recreational Services Assessment on all residents.
- Reassess and develop care plans as needed.
- Participate in Interdisciplinary Team meetings (e.g. care planning meetings).
- Maintain daily statistics.
- Maintain MDS accurately and within set time frames.
- Complete quarterly Progress Note form on all assigned residents.
Qualifications:
- National Council for Therapeutic Recreation Certification (CTRS), Required
- BA degree in Therapeutic Recreation and/or related field
- Excellent planning, writing, organization, and time management skills
- Ability to communicate with families, professionals, and co-workers
- Demonstrate positive patient/family interaction skills is essential.
- Working knowledge of computer skills including word and excel.
