KENOSHA HUMAN DEVELOPMENT SERVICES, INC.3536 52nd Street Kenosha, WI 53144Division/Department: Homeless Assistance ServicesLocation: Main OfficeJob Title: Referral and Coordinated Entry CaseworkerReports To: Homeless Services Program ManagerType of Position: Full-time FLSA Status: Exempt Schedule: 40 hours per week or as demanded by the jobFor more information, please click on this link: Referral and Coordinated Entry Caseworker 12-26-2024POSITION SUMMARYThe Referral and Coordinated Entry Caseworker is responsible for responding to and following up with all homeless assistance referrals, authorizing motel voucher placement and providing short-term case management services to people in the motel voucher program.RESPONSIBILITIES1. Respond and connect people experiencing literal homelessness and at risk of homelessness to appropriate services based on assessed need a. Respond to all Homeless Assistance Services referrals. b. Ensure people placed in motels are provided food or food vouchers c. Complete the EMVA Intake and Coordinated Entry Process with all persons entering the motel d. Assist people placed in motels develop a plan for housing and motel exit e. Refer people to community resources to help meet identified needs f. Complete progress notes for client contacts in the Client Tracking System g. Complete Coordinated Entry follow-ups on a weekly basis h. Document client-specific contact in the Client Tracking System and Mr. Cool i. Document non-client specific activities in the Homeless Assistance Services (HAS) Documentation System j. Enter and maintain data in the Homeless Management Information System (HMIS) as required k. Create and maintain a file for each client served in the Emergency Motel Voucher Program l. Verify and submit motel payment requests on a weekly basis and more often when possible m. Authorize motel vouchers for eligible person during business hours when shelters are at capacityQUALIFICATIONS: Bachelor's degree in a human services related field and/or experience working with individuals and families experiencing homelessness and/or other issues that put individuals at risk of housing instability, preferred.1. Ability to effectively communicate, both written and verbally, in the English language2. Must have reliable transportation and valid driver's license with good driving record3. Must carry own auto insurance with at least 100,000/300,000/100,000 coverage4. Ability to maintain records and document case notes daily5. Ability to give and accept feedback as a part of a team6. Ability to traverse within the workspace and community frequently7. Be willing to learn and work with the treatment philosophies8.
