POSITION TITLE: Referral CoordinatorREPORTS TO: Director of SitesCLASSIFICATION: Non-ExemptCareSTL Health is seeking Referral Coordinators. The Referral Coordinators performs administrative tasks to make recommendations to people in need of services, as well as relaying important information to companies and organizations. While this position primarily serves healthcare entities, Referral Coordinators may also work in children's services, welfare, and educational settings.ESSENTIAL FUNCTIONS:The following information is considered the definition of essential functions but does not restrict the tasks that may be assigned. The Referral Coordinator may be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons. Other duties may be assigned.* Coordinates internal and external referral authorizations and tracks specialty care referral to show trends of patient's compliance/noncompliance with providers' recommendation for specialty care to ensure timely access to care of our patients to outside facilities* Provide support to hospitals, public and private insurers so that referral authorization can be obtained in timely manner to ensure the best health outcome for our patients and their families* Actively participates in the performance improvement committee and performs as a cohesive team member as well as communicates well with public and private insurers and hospitals* Provides monthly report on status of all referrals on an ongoing basis and actively tracks patient's referral to outside agencies and maintain referral log for tracking in EMR* Coordinates multiple agency services for the benefit of the client while assisting patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance)OTHER FUNCTIONS:* Establishes linkages among service providers and possesses the ability to function independently* Reinforces client responsibility for independent compliance with keeping appointments* Evaluates client progress in accessing appropriate medical care and other needed services* Maintains and update technical knowledge and skillsJOB REQUIREMENTS AND QUALIFICATIONS:Education:* High School Diploma or GED* Graduate of an accredited school of Medical Assistant preferred* Current CPR CertificationExperience:* 1-2 years of experience in a community health center and/or medical office settingAdditional Information:* Clinic hours are Monday - Friday 8 a.m. to 5 p.m.* 11 paid holidays per year.* Full benefits package.* Public Service Loan Forgiveness.* We believe that health care is right and not a privilege.
