DESCRIPTIONThe Referral Coordinator is responsible to Facilitate referrals by processing internal, outgoing, and incoming referral entries. The Referral Coordinator will be responsible for working assigned incoming and outgoing referral work queues, schedule patients, attach referral to patient appointments, obtain authorizations, proactively investigate assigned department schedule, and verify all authorizations have been done.RESPONSIBILITY* The Referral Coordinator schedules, problem solve, communicates, and coordinates referral appointments with outside specialists upon receipt of the Referral Order from designated. Reviews, screens, and distributes incoming faxes accordingly.* Tracks and enters all referral requests in the Referral Management System and Insurance Electronic System daily.* Coordinates with the clinical staff to track the status of all provider reports on patients referred to hospitals, outside clinics and Specialty Providers and follows up for reports/records not received on referred patients.* Maintains an accurate records of external referral patient's appointments that are kept, failed, and status in the Referral Management System for outstanding, provider reports that are (30 days or more old), and follows-up with the appropriate provider.* Ensures that all referral forms are sent to health records in a timely manner, after assigned actions are completed.* Provides Intensive tracking and follow-up on all patient's referral orders. Documents efforts to reschedule patients and provide documentation.* Routinely makes reminder phone calls, reminder to referral patients in advance of appointment with outside providers.* Responds to in-house provider and clinical staff questions, requests, and concerns regarding the status of patient referrals, care coordination or follow-up status.* Ensures that referrals are addressed in a timely manner.* Verifies insurance information for referral purposes and process insuranceQUALIFICATIONS* High School Diploma* Minimum 2 years' experience in a medical setting (required)* Epic experience (preferred)* Working knowledge of Microsoft Office applications* Practice Management Software Experience.* Excellent communication skills.* Knowledge of using Epic System* Medical terminology knowledge preferred.PREFERRED REQUIREMENTS* Bilingual - English/Spanish or English/CreoleLICENSES, CERTIFICATION OR REGISTRATION: NoneAny appropriate combination of relevant education, experience and/or certifications may be considered.Education needs to come from an accredited school or educational college/institution.Empower U , Inc. is an equal opportunity employer.
