Join our healthcare team and utilize your organizational skills and strong communication abilities to manage referrals, coordinate care, and contribute to enhancingthe patient experience.Job Summary: Referral Coordinators manage patient referrals, facilitate communication between healthcare providers, and ensure smooth transitions of care. This role demands strong organizational skills, attention to detail, and effective communication for optimizing patient care and operational efficiency within healthcare settings.Responsibilities & Essential Function* Responsible for facilitating patient referrals and obtaining pre-authorizations.* Completes pre-authorization in accordance to patient's payer source requirements.* Confirms specialist/facility is in-network according to payer source.* Facilitates referral scheduling* Unscheduled referrals - assists to coordinate or to directly schedule specialty appointment, under direction of ordering Provider ensures PHI is sent to specialist, document all activity within order & PHI log, and help to identify patient barriers and escalate to case management as needed.* Scheduled referrals - ensure patient compliance with scheduled appointment, follows measures outlined within workflow for missed, cancelled or declined appointments.* Assists in acquiring specialty notes once appointment has been kept.* Provides superb customer service to all customers internally and externally* Contacts patient via phone or in writing to advise of referral status.* Provide timely response to patients on the status of obtaining prior authorization from insurance companies.* Responds to patient inquires and concerns with regards to the pre-authorization and referral appointment process.* Refers patients with identified barriers, i.e. transportation, language, etc. to Case Management staff to help address patient needs.* Coordinates all diagnostic orders in conjunction with workflow.
