- Current State Registered Nurse License
- Minimum of three (3) years of experience in Hospice nursing management and/or QAPI management, strong Hospice experience, preferably with Quality/QAPI experience, and two (2) years medical-surgical experience prior to hospice.
- Certified in Hospice and Palliative Care preferred
- The ability to communicate and train hospice staff on CHAP, State and Federal hospice Regulations and complete knowledge and understanding of the functions of a hospice organization.
- Ability to train and teach staff members in the use of Home Care Home Base (HCHB), Allscripts, databases, etc.
- Good working knowledge of Local Coverage Determinants for Hospice (multiple states)
- Good working knowledge of the Medicare Conditions of Participation (CoPs)
- Good working knowledge of the Community Health Accreditation Partners (CHAP)
- Ability to travel

- Provides direction of the team/group of each agency as it relates to patient services, agency operational processes and efficiencies, and quality control.
- Responsible for orientation/onboarding and training of Patient Care Managers in all aspects of the role and responsibilities.
- Provides ongoing education and training in managing pharmacy, DME, Medical Supplies utilizing companies formularies, best practices.
- Responsible for oversight of orientation of all clinical staff, ongoing staff training and education, and training with HCHB.
- Oversight of each agency to ensure that all offices are in compliance with CHAP, federal and state regulations. Responsible to ensure agency survey readiness.
- Support agency during internal and external surveys to include development and implementation of the plan of correction if indicated.
- Provides education and training for areas of improvement with clinical processes, clinical documentation, and patient care.
- Coordinates and schedules training for the hospice sites to compliment the areas of improvement seen through the auditing process.
- Works collaboratively with each site to review, assess and implement plan(s) utilizing best practices to improve CAHPS scores to meet or exceed the national average.
- Works directly with Chief Clinical Officer and Chief Compliance officer to develop, evaluate, review, revise and implement policies and procedures and areas of improvement.
- Works directly with RVP, Chief Clinical Officer, and Chief Compliance Officer to evaluate, review, revise and make recommendations on improvement with our EMR (HCHB).
- Assist in development of tools and education for HCHB for staff and external customers utilizing HCHB.
- Keeps up to date and maintains a working knowledge of current CHAP, state and federal regulations.
- Keeps up to date on new methods, products, trends, in hospice care.
- Collaborates and advises the RVP, Chief Clinical Officer and Chief Compliance Officer on the clinical and compliance needs of each office.
- Provides day to day support via phone, email or on site to ED, clinical leaders, and their staff concerning outstanding issues of patient care, appropriateness and compliance.
- Provide training and oversight on live discharge process and approval and NTUC's at each site to ensure all sites are managing according to company policy. Works with agency to ensure monitoring of LLOS via internal audit and review process.
- Maintains a working knowledge of current Medicare/Medicaid requirements for hospice reimbursement. Ensure agency is in compliance with billing requirements.
- Maintains patient confidentiality requirements.
- Track and trend reports; initiate and update tracking spreadsheets; readily develop and submit new reports as requested.
- Conduct Employee Performance Evaluations.
- Communicates the mission, ethics and goals of the organization.
- Timely submission of all documentation
- Performance of other duties as required
