Registered Dietician - Per Diem

This position has the overall responsibility to provide lifestyle management for people with chronic disease as designated by the Medical Director, Administrative Director and Practice Manager of CCHL/CCMBS. The Registered dietitian is responsible for the provision of direct patient care to a designated patient or group of patients.

Roles and Responsibilities:

  • Identify patient's goals and needs. Provide quality care by assessing and interpreting evaluations; determine care plans and manage patients care.
  • Phone and voicemail management.
  • Observe data collection, care coordination, planning, implementation and evaluation of the patient's physical, psychological, emotional, educational and spiritual needs.
  • Collect patient from waiting room and escort patient to the designated room.
  • Document patient care services and charges by charting in patient's electronic medical record.
  • Maintain professional and technical knowledge by attending continuing education webinars and in-services; and reviewing professional journals.
  • Cooperative creation and development of outpatient nutritional counseling and nutrition education programs for the Cayuga Center for Healthy Living and the Cayuga Medical Center community.
  • Follow protocols and pathways as appropriate to maximize documentation, efficiency, and timely charting.
  • Outpatient nutrition assessment and education with a focus on Bariatrics, Diabetes, weight management and cardiac nutrition in both individual and group settings.
  • Nutrition evaluation and counseling of Bariatric program clients, and participation in Bariatric team programming/meetings.
  • Is flexible in assuming other appropriate responsibilities not noted above as appropriate.

Preferred Skills and Experience:

  • Strong oral and written communication skills
  • Ability to assess situations and make decisions
  • Must complete yearly mandatory safety in-services and/or trainings
  • Must complete yearly Health Stream e-Learning Courses
  • Must adhere to patient safety Red Rules .
  • Licensure: Registered Dietitian and CDN.

    Medicare Provider with National Provider Identifier upon employment.

Location and Travel Requirements:

  • Onsite preferred but remote is acceptable

Pay Range Disclosure:

  • $62,400 to $83,200 per year
