Required Qualifications?(as evidenced by an attached resume):
Bachelor's Degree (foreign equivalent or higher) in nutrition or related degree program. Completion of a Dietetic Internship.
Preferred Qualifications:
Master's degree (foreign equivalent or higher). Registered Dietitian or RD-eligible. Experience working with individuals to provide personal and group counseling on improving diet and lifestyle behaviors to make a positive impact on health and prevent disease progression.?Experience in medical documentation using an electronic medical record system.?Candidates who have previously interned with the Word Trade center health and Wellness Program.
Brief description of duties:
The clinical Registered Dietitian will work for the WTC (World Trade Center) Health Program's Long Island Clinical Center of Excellence at Stony Brook Medicine as part of the clinical team.?The role of the clinical Registered Dietitian will be to assess the nutritional needs of our patients who come to the WTC Health Program for annual health monitoring visits and treatment for WTC-related illnesses and to counsel patients to establish healthier diets and eating habits.?The clinical Registered Dietitian will work with case managers, providers, nurses, and pharmacists in a collaborative care approach. They will be required to document individual nutrition plans in the patient's medical record and collect data for continuous quality improvement and quality assurance purposes in order to assess the impact of the nutritional counseling programs on overall health of our patients.
Duties:
* Evaluate needs of the WTC responder population with regard to nutritional programming and?assist with developing appropriate programs that will educate members on the impact nutrition has on disease progression and prevention.
* Assess individual patient nutritional status and needs, develop individualized nutritional plans for patients, counsel and motivate compliance, and document in patients' medical records.
* Collect data and writing reports. Evaluate effectiveness and provide insight and recommendations.
* Work collaboratively with clinical and member services teams to provide excellence in care, including scheduling appointments and educating members on the program.
* Work in collaboration with case management team, care providers to provide administrative and clerical support.
* Assist with creating and maintaining spreadsheets and databases for nutrition appointments and communications.
* Produce and maintain thorough, accurate and timely documentation, including notes in the Electronic Medical Records (EMR) system.
* Documentation of interactions with members with details about services provided and topic and outcome of call.
* Assist with member pre-screenings assessments and face-to-face assessments.
* Regularly collaborate with case management, coordinate, and communicate with members, family members, and interdisciplinary team members.
* Assist nutrition faculty on research projects involving the WTC responder population.
* Attend and participate in meetings and training sessions, as required.
* Other duties as assigned.

The incumbent must be willing to work and travel between the Nassau and Commack clinic locations. Occasional Evenings and Saturdays will be required.
