Eddy SeniorCare is an exciting, innovative model of care for adults 55+, located in Rotterdam and Latham,NY. This position is for a full time Dietitian in our Rotterdam location. Our Program of All-Inclusive Care for the Elderly provides a team-based model of care including a physician, RNs, LPNs, Dietitians, Home Health Aides, physical/occupation/speech therapists, social workers, transportation, a day center team, and more! This is a truly unique program our region is fortunate to have access to, with only 154 PACE programs across the country!

Our Registered Dietitians play a crucial role in our unique model of care. The dietitians work closely with the participant and interdisciplinary team to conduct comprehensive nutrition assessments and create personalized care plans. They offer guidance to our participants, so they can manage their own nutritional goals in the community. Our dietitians provide education and counseling to participants and family members on a variety of health issues like weight management, chronic kidney disease and diabetes. They develop interventions such as nutritional supplements and home-delivered meals and collaborate with outside agencies such as Meals on Wheels and food pantries. They are actively engaged with the Physician and Nursing team to help minimize preventable infections, weight loss and wounds. PACE dietitians work closely with day center staff to oversee meal service and provide education and in-service training to staff members. They participate in professional activities and continuing education programs through Unidine and Trinity Health.

As a Registered Dietitian at Eddy SeniorCare, you focus on the participants who need you the most. The dietitian evaluates the need for patient follow ups, and makes appropriate referrals as needed. Your day is not directed by productivity standards, but rather by a team goal of helping seniors delay or avoid nursing home placement, and remain safely in their homes as long as possible.

Key Responsibilities:

  • Provides medical nutrition therapy including nutrition assessment, diet modification, nutrition education, and intervention for the patient population
  • Follows all guidelines as outlined within the Company Diet Manual and the Clinical Nutrition Policies & Procedures Manual
  • Complies with all regulatory standards to include federal, state, and accrediting agencies while adhering to facility confidentiality, HIPAA regulations, and patient rights policies
  • Trains and mentors patient services staff and interns as applicable
  • Participates in patient satisfaction programs, departmental meetings, and facility wide Quality Assurance/Performance Improvement programs

Qualifications:

  • Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR)
  • Licensed by the State Dietetics Licensing/Credentialing Board, in states where required
  • One (1) year of long term care experience, preferred
  • Possess the necessary skills to effectively utilize Microsoft office applications, electronic medical record & diet office systems, and nutrient analysis programming
