To provide high quality medical nutrition therapy and work as an active member in the multidisciplinary team. To be a resource on nutritional information to physicians and staff and represent Food and Nutrition Services throughout the Kaweah Health.

JOB RESPONSIBILITIES
Essential
Performs assessment and screening for patients designated as a high risk for optimal nutrition. Develops nutritional treatment plans in coordination with the interdisciplinary team and in collaboration with the patients and families as able. Follows up such patients to evaluate progress towards goals.
Formulates individual Nutrition Care Plan and MDS plans for long term care patients to meet patient's current nutrition needs.
Provides nutrition information and recommendations to physicians regarding patient's nutritional status in a timely manner. Charts appropriately and in a timely manner to the patient's medical record.
Performs or coordinates nutrition education for patients and / or patient's caregivers, and charts appropriately to the patient's medical record. Tailors nutrition education to the needs of the patient and their family (ex: medical needs, cultural preferences, patient readiness, etc).
Participates in Continuum of Care Meetings, Interdisciplinary Meetings, Unit Rounds and family conferences as a nutrition consultant on assigned floors.
Continuously looks for ways to improve nutritional care to patients. Acts with others to monitor current levels of service, plan new or improved services, implement the plan, and reassess, educate, and make changes necessary for excellent care of patients.
Participates in development of Standards of Care for clinical nutrition and participates in continuous quality improvement.
Addendum (essential for specific dept)
Renal Dietitian also responsible for the following: Gathers research materials with regards to renal nutrition, compiling patient information, diet handbook, and educational materials. Provides in-service to staff regarding aspects of renal nutrition.
Peds/NICU Dietitian also responsible for the following: Oversees planning, preparation, storage, and dispensing of pediatric/infant formulas. Provides nutritional counseling to the pediatric/infant parent or guardian.
Diabetic Education, also responsible for the following: Coordinates scheduling of patients with RN/CDE, and social services in Diabetes Center. Participates in monthly review meetings with Medical Director of Diabetes Center. Participates or leads local diabetes educator meetings, diabetes support group meetings, diabetes workshops, screenings and any outreach activities. Provides staff education programs to ensure staff is competent in providing diabetes education to patient and/or their family members.
Long Term Care/Rehabilitation/Mental Health also responsible for the following: Responsible for compliance and oversight of foodservice areas.
Additional
Knowledgeable of community resources to help meet patient's nutritional needs.
Participates in various task forces or committees in which nutrition care or education is a factor of care. Acts as the resource person and/ or the owner to effect change.
Represents clinical nutrition and the hospital Kaweah Health at various meetings in the community.
Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs. Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.
Performs other duties as assigned.
Pay Range
$36.00 -$54.00
