Responsibilities:
+ Assesses the nutrition status of patients referred with the use of a 24-hour recall or similar evidence-based methods, uses a whole-person approach in nutrition assessment.
+ Reviews the electronic medical record (EHR) and any lab data available to assist in assessment.
+ Records information in the EHR using the appropriate intake forms and templates.
+ Counsels, coaches, and educates patients on appropriate nutrition guidelines relating to their condition.
+ Uses motivational interviewing and shared decision making with patients to help them reach their health goals.
+ Works with other members of the patient's health care team as needed to support the patient achieving their health goals.
+ Provides case management services to support improving outcomes of at-risk populations.
+ Supports overall wellness-related initiatives as needed to improve both NHC patient population health and NHC employee wellbeing.
+ Maintains a presence in the clinical area by:
+ being available for nutrition education,
+ being assistance for providers and staff,
+ being a warm hand-off of patients in need of nutrition support and
+ maintaining consistent nutrition/wellness resources across the practice.
+ Maintains confidentiality per HIPAA regulations.
+ Other duties as assigned.
Requirements:
+ Must possess a BS or MS degree in the field of dietetics from an accredited college or university with completion of a certified internship program.
+ Must possess a current NYS Registered Dietitian license.
+ Preferably to have two (2) or more years' experience as a clinical or community-based dietitian.
+ Preferably to have two (2) or more years' experience as a clinical or community-based dietitian.
+ Must be able to lift & carry 10 pounds regularly, more occasionally.
+ Must be able to walk/sit/stand/bend/twist intermittently throughout the workday.
Compensation:
$61,500 - $64,000 annual salary (based on full time, 40 hour work week)
Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications and other position related components.
